Pulmonary arteriovenous malformation

Telangiectasias are collections of dilated capillaries that are usually of no clinical significance (Milandre et al. 1987). They may be associated with hereditary hemorrhagic telangiectasia (the Osler-Weber-Rendu syndrome), but this is more likely to be associated with neurological complications from a pulmonary arteriovenous malformation with right-to-left shunting, such as cerebral hypoxia, brain abscess, paradoxical and septic embolism, or from an associated intracranial arteriovenous malformation or aneurysm (McDonald et al. 1998). [Pg.98]

Fig. 3.9a-d. A standard 7-F gonadal guide catheter for occlusion of the left spermatic or ovarian vein (Cordis Inc., Miami, FL) is demonstrated in (a) and in (b) the 7/5 pulmonary guiding catheter and inner catheter for occlusion of pulmonary arteriovenous malformations is shown. Once the ovarian or internal spermatic vein are catheterized, any standard 100-cm 5-F endhole multipurpose catheter is advanced over a Bentson wire deep into the spermatic or ovarian vein where sclerosants and coils are usually placed. A standard 6-F RDC (Cordis Inc., Miami, FL) guide catheter for visceral embolization is demonstrated with a coaxial 4-F catheter in (c) and in (d), a triaxial system is demonstrated with an inner 0.021 lumen microcatheter... [Pg.40]

Rare complications as have been reported in literature are aortic and bronchial necrosis [58], bronchial stenosis [59], unilateral diaphragmatic paralysis [60], pulmonary infarction (especially in patients who have suffered pulmonary artery embolism), left main bronchial-esophageal fistula [61], and non-target embolization (colon, coronary and cerebral circulation) [62]. Especially the newer spherical embolic materials (tris-acryl gelatin) can traverse from the bronchial into the pulmonary circulation, and then through unoccluded pulmonary arteriovenous malformations into the systemic circulation [41]. [Pg.275]

Pulmonary arteriovenous malformations are caused by abnormal communications between pulmonary arteries and pulmonary veins, which are most commonly congenital in nature [4, 20]. Although these lesions are uncommon, they are an important part of the differential diagnosis of common pulmonary problems such as hypoxemia and pulmonary nodules. These abnormal communications have been given various names including pulmonary arteriovenous fistulas, pulmonary telangiectases, and pulmonary arteriovenous malformations [20,66). [Pg.279]
