Because Treponema pallidum is sensitive to penicillin G, this antibiotic is the drug of choice for treatment of syphilis and syphilitic eye disease (see Table 11-1). Many means for prolonging the sojourn of the antibiotic in the body and thereby reducing the frequency of injections have been explored. Therapy of syphilis with penicillin G is highly effective. Primary, secondary, and latent syphilis of <1 year s duration may be treated with penicillin G procaine (2.4 million units per day intramuscularly), plus probenecid (1.0 g/day orally) to prolong the tj for 10 days or with 1—3 weekly intramuscular doses of 2.4 million units of penicillin G benzathine (three doses in patients with HIV infection). Patients with neurosyphilis or cardiovascular syphilis typically receive intensive therapy with 20 million units of penicillin G daily for 10 days. Since there are no proven alternatives for treating syphilis in pregnant women, penicillin-allergic individuals must be acutely desensitized to prevent anaphylaxis. [Pg.736]

B. Pharmacokinetics Penicillins vary in their resistance to gastric acid and therefote vary in their oral bioavailability. They are polar compounds and are not metabolized extensively. They are usually excreted unchanged in the urine via gloinemlar filtration and tubular secretion, the latter process being inhibited by probenecid. Ampicillin and nafcillin are excreted partly in the bile. The plasma half-lives of most penicillins vary from one-half to 1 hour. Procaine and benzathine forms of penicillin G are administered intramuscularly and have long plasma half-lives because the active drug is released very slowly into the bloodstream. Most penicillins cross the blood-brain barrier only when the meninges are inflamed. [Pg.375]

A study in patients given intramuscular procaine benzylpenicillin 2.4 or 4.8 million units with or without probenecid 2 g found the peak serum levels were higher in patients given probenecid, but because of wide interpatient variation, possibly associated with differences in the release of penicillin from the injection sites, the exact potentiating effect of probenecid could not be determined. However, another study in men and women given procaine benzylpenicillin 2.4 million units and 4.8 million units, respectively (for uncomplicated gonorrhoea), found treatment failure after 1 week in 15.4% of men and 10.4% of women. Failure rates were reduced to 1.8% and 3.7%, respectively, when oral probenecid 1 g was given with the penicillin."... [Pg.325]
