So had the safety culture changed Yes The entire culture moved from a reactive mode into a proactive safety mode with the implementation and maintenance of a world s best safety management system led by the management teams. They provided the resources and allowed for the development of those resources, and also participated in the development process by attending the training and getting involved in safety inspections, committees, and recognizing employees. 

Recognizing signals and early warnings through the use of proactive safety indicators will reduce the risk of major accidents. There is no need for a discussion what is lead and what is lag, but there is a need to develop and implement useful... 

For proactive safety management to work, it is necessary to foresee what can happen with acceptable certainty and to have the appropriate means (people and resources) to do something about it. That in turn requires an understanding of how the system works, of how its environment develops and changes, and of how functions may depend on and affect each other. This understanding can be developed by looking for patterns and relationships across events rather than for causes of individual events. To see and find those patterns, it is necessary to take time to understand what happens and not spend all resources on firefighting. 

The focus of this chapter has been on proactive application of these analytical methods such as safety audits, development of procedures, training needs analysis, and equipment design. However, many of these methods can also be used in a retrospective mode, and this issue deserves further attention in its own right. Chapter 6 describes analytical methods for accident investigations and data collection. 

For this reason, it makes sense to examine the costs and benefits of making improvements in your environmental, health, and safety programs. Make the HSE area part of your company s planning process, so that change can be proactive rather than reactive. Develop a schedule of implementation, as it is difficult to tackle all of these areas at once. You will need to decide whether to develop the expertise internally or seek outside assistance to devise and accomplish your plan. Training is readily available if you want to do it yourself, and there are many capable consulting firms ready to assist.
