Pneumonitis caused

Headache, tachypnea, dizziness, confusion, and chest pain. The casualty may also experience palpitations, dyspnea on exertion, drowsiness, lethargy, hallucination, agitation, nausea, vomiting, diarrhea, and coma. If metal carbonyls have been released, there may be complaints of irritation of the eyes, mucous membrane, and respiratory system. Inflammation of lung tissue (pneumonitis) caused by metal carbonyls can may be delayed 12-36 hours. They may also cause injury to the liver, kidneys, and lungs as well as degenerative changes in the central nervous system.

Type III reactions are the result of antigen-antibody (IgG) complexes that accumulate in tissues or the circulation, activate macrophages and the complement system, and trigger the influx of granulocytes and lymphocytes (inflammation). This is sometimes referred to as the Arthrus reaction and includes postinfection sequelae such as rheumatic heart disease. Farmer s lung, a pneumonitis caused by molds has been attributed to both type III and type IV, and some of the late phase response (4-6 hours after exposure) in asthmatics may be the result of Arthrus-type reactions.

An evaluation of the lung-to-liver ratio is necessary to estimate the likelihood of a pneumonitis caused by radiation. A first follow-up is reasonable 2 weeks after discharge (assessing blood values, looking for pneumonitis). Estimation of the treatment s success (blood values, tumour marker, CT, PET, if so static liver scintigraphy) (Fig. 2.8.2) and a decision on further I-lipiodol treatments should be made within 3 months of the first treatment.
