Preoperative assessment of coronary status and cardiovascular function is based on clinical evaluation, noninvasive and invasive diagnostic testing. Physical performance is evaluated according to simple exercise tasks (e.g. walking up stairs), and is quantified by grading their metabolic equivalent (MET, table 1). At a performance of 4 MET or more, the patient is usually fit for vascular access surgery in locoregional anesthesia (ERA) without further cardiac evaluation [6].

Baseline assessment should define therapeutic goals and document cognitive status, physical status, functional performance, mood, thought processes, and behavior. Both the patient and caregiver should be interviewed. 

A validation report is a written document that cross-references the validation protocol, summarizes the results obtained, describes any deviations observed, and draws the necessary conclusions, including recommending changes required to correct deficiencies for the qualification and validation performed [5]. In this report it is required to present both the results and conclusions and the secure approval of the study. The report should include a summary of the procedures used to clean, sample, and test as well as the physical and analytical test results or references for the same. The conclusions regarding the acceptability of the results should also be included. Other information would be the status of the procedures being validated, any recommendations based on the results, or any relevant information obtained during the study. These include, re validation practices (if applicable), the approved conclusions, and any deviations of the protocol that might have occurred. In cases where it is unlikely that further batches of the product will be manufactured for a period... 

The inspection and test status of the product is identified by markings, authorized tags, labels, routing cards, inspection records, test software, and physical location, which indicate the conformance or nonconformance of the product with regard to inspection and tests performed.

The identification of inspection and test status is maintained, as necessary, throughout production and installation of the product to ensure that only product that has passed the required inspections and tests is dispatched, used, or installed. 

The physician will take a medical history and perform a physical examination. The details of the mental status examination will be greater from a psychiatrist than a primary care physician, and the primary care physician will probably conduct a more thorough physical examination than most psychiatrists. In common, their consultations will usually include the elements in Table 9.6. 

A thorough mental status examination, physical and neurologic examination, a complete family and social history, vital signs and laboratory workup (complete blood count, electrolytes, hepatic function, renal function, electrocardiogram [ECG], fasting serum glucose, serum lipids, thyroid function, and urine drug screen) should be performed prior to treatment.
