Verify the patient s allergy history and the type of reaction experienced. Attempt to discern between true allergy and adverse event. (3-Lactam-allergic patients may receive clindamycin, vancomycin, or other antimicrobials. Crossreactivity between penicillin allergy and cephalosporins is low but cephalosporins should be avoided in patients with a history of anaphylaxis to penicillins. [Pg.1237]

Because of cross-reactions between cephalosporins and penicillins, caution should be used when prescribing cephalosporins to patients with penicillin allergy. If a patient had anaphylaxis, angioedema, or urticaria fol-... [Pg.533]

Cephalosporins are sensitizing and may elicit a variety of hypersensitivity reactions that are identical to those of penicillins, including anaphylaxis, fever, skin rashes, nephritis, granulocytopenia, and hemolytic anemia. However, the chemical nucleus of cephalosporins is sufficiently different from that of penicillins so that some individuals with a history of penicillin allergy may tolerate cephalosporins. The frequency of cross-allergenicity between the two groups of drugs is uncertain but is probably around 5-10%. However, patients with a history of anaphylaxis to penicillins should not receive cephalosporins. [Pg.993]

A large national study by venereal disease clinics in the USA, including four cooperative surveys conducted at 5-year intervals (1954,1959,1964, and 1969). The study included data from 94655 patients unselected with regard to a history of penicillin allergy. The frequency of anaphylaxis was 0.055%, including one death (234). [Pg.487]

In a retrospective study the frequency of systemic anaphylaxis to cefaloridine, cefalotin, or cefalexin was two out of 9388 patients (0.02%) without a history of penicillin allergy, and two out of 450 patients (0.4%) with a history of peniciUin allergy (152). In the first group, two of the 1983 patients treated with cefalotin accounted for the adverse event. [Pg.693]

Cross-reactivity between pencillins and cephalosporins ranges from 5% to 10%. Even though some patients with a history of penicillin allergy may tolerate cephalosporins, patients with a history of anaphylaxis to penicillin should not receive cephalosporins. [Pg.111]

T cells control these learned responses and decide which tools to use in the reaction. Sometimes they choose several different tools at once, and multiple reactions ensue, such as when a person becomes sensitized to penicillin and has not only anaphylaxis but hemolytic anemia and serum sickness. There are different types of T cells, and they communicate either directly with other cells or by chemical messages called cytokines. The pattern of cytokines released is one way T cells have of determining which kind of response will occur. They are broadly called Thl andTh2 responses, with Thl mostly responding to infections and Th2 often producing allergy or asthma. [Pg.820]

Patients with a history of allergies, especially to penicillin, are at increased risk for developing a severe hypersensitivity reaction, marked by severe pruritus, angio-edema, bronchospasm, and anaphylaxis. [Pg.204]

Nephrotoxicity may occur, especially in patients with preexisting renal disease. Severe hypersensitivity reaction including severe pruritus, angioedema, broncho-spasm, and anaphylaxis, particularly in patients with a history of allergies, especially to penicillins, may occur. [Pg.215]
