It is noted that conditioned withdrawal responses are less pronounced than with opiates yet, withdrawal appears to have a phase-specific relation to the reemergence of cue-sensitive responses that deserves further research. 

Prior to routine screening of blood products in the early 1990s, the primary route of transmission of the HCV was blood transfusions, when the risk was 0.02% per unit transfused. Since then, the risk has decreased significantly (0.001% per unit transfused).13 Today, intravenous drug users utilizing contaminated paraphernalia are responsible for most HCV transmission. Other populations at risk for acquiring HCV are listed in Table 21-1. Approximately 10% of the individuals infected with HCV have no identifiable risk factors. 

The use of contaminated needles or other injection-related paraphernalia by drug abusers has been the main cause of parenteral transmissions of HIV. 

Sometimes a person with a drug problem will distance him- or herself from the family by seeking seclusion in a private area of the house (like a basement or a shed) in order to use drugs. In some cases people with drug problems may make this location off-limits to other family members, or may insist on time alone at specific times of the day. Loved ones may discover drug paraphernalia by accident in those areas of seclusion or in other places frequented by the person. 

September 11 Comedian Thomas Chong, half of the duo of Cheech Chong, is sentenced to nine months in jail for selling marijuana pipes, bongs, and other drug paraphernalia. The duo had been famous for its portrayal of stoned marijuana users. 

In 1988, the U.S. Consumer Products Safety Commission banned the sale of butyl nitrite. But manufacturers kept one step ahead of federal regulatory agencies. Methadone is given orally (different from the usual means of abuse), and thereby also substitutes for the paraphernalia that accompany illicit drug use. Methadone helps relieve the craving for more drug and delays the appearance of withdrawal symptoms as long as it is in the body. Doses are gradually decreased. If the dose of morphine that was abused is known, then 1 mg of methadone can substitute for 4 mg of morphine. Otherwise, a dose of 10-50 mg a day is generally used, and can be reduced by 20% per day. 

Internet sales of whippet cartridges are a growing problem that has resulted in the death of at least one nitrous user. Sold under the guise of whipped cream propellants and erotic aids, whippets are frequently sold alongside crackers, balloons, and other drug paraphernalia that make their intended use fairly clear.

Trace versus bulk samples A trace sample is considered to be one, which is barely visible to the naked eye. Samples such as this will be present on the insides of reaction vessels, scales, knives, and other drug paraphernalia. In many cases, trace samples are swabbed from these items and are analyzed using confirmatory techniques first. Such samples are very easily contaminated and significant care should be taken in their analysis. Bulk samples require a sampling methodology and are generally subjected to presumptive tests and TLC followed by confirmatory tests.
