Testing in humans to obtain clinical data demonstrating these qualities in support of a BLA must be conducted pursuant to an investigational new drug appHcation (IND). The IND is the functional equivalent of the IDE that governs clinical trials of devices. As with other medical products, clinical testing is typicaUy conducted in multiple phases, with the earhest phases primarily intended to demonstrate safety, and later phases intended to address both safety and efficacy considerations.

When it approved the New Animal Drug AppHcation (NADA) of formalin, FDA ruled that use of formalin for fisheries was safe for humans and the environment. They ruled that effluents from fish treatments at 250 mg/L should be diluted 10 times and from egg treatments 75 times if 1,000 —2,000 mg/L were used (10,11). Before registering the compound, FDA also addressed carcinogenicity by stating it was not concerned about human exposure from either water or fish treated with formalin. The U.S. Fish and Wildlife Service (USFWS) has procedural guidelines that should protect workers from harm fill levels of formalin. Calculations based on treatment levels demonstrated that a fishery worker is exposed to not more than 0.117 mg/L formalin in the air, well below the levels estabUshed by the U.S. Occupational Safety and Health Administration to protect workers. 

An Investigational New Drug (IND) application to the FDA seeks permission for a human chnical trial to be conducted. An IND apphcation is detailed under 21 CFR Part 312. The process for an IND is summarized in Fig. 8.2.

Phase II trials in breast cancer and head and neck cancer were underway in Japan by November 2005 and in brain cancer by December 2005. By July 2005, a phase III trial in solid tumors had started. In February 2006, the company listed the drug in phase III trials for NSCLC. AstraZeneca plans to file Vandetanib for marketing authorization apphcation (MAA) in Europe and new drug application (NDA) in the USA not earlier then 2007.

We had predicted in our 1-hydroxyindole hypothesis that the introduction of a hydroxy or an alkoxy group onto the 1-position of biologically inactive indole compounds makes them become biologically active compounds [1-7]. In addition, the apphcation of the idea to the known biologically active indoles has their original activities improved or modified for the discovery of new drugs [1-7]. 

AppHcations for approval of new drugs and medical devices are referred by PMSB to the Central Pharmaceutical Affairs Council (CPAC) to obtain its recommendation. The CPAC, in turn, is advised by the Pharmaceutical and Medical Devices Evaluation Center (PMDEC), an expert body organized in July 1997 to evaluate the quality, efficacy, and safety of medical products administered to humans. Specific authority within PMSB to approve recommendations received from CPAC regarding the discrete aspects...
