Propafenone Metoprolol

A 24-year-old woman briefly lost consciousness and had nausea and vomiting several minutes after smoking marijuana. She had hyporeflexia, atrial fibrillation (maximum 140/minute with a pulse deficit), and a blood pressure of 130/80 mmHg. Echocardiography was unremarkable. Within 12 hours, after metoprolol, propafenone, and intravenous hydration with electrolytes, sinus rhythm was restored.

As parecoxib is rapidly metabolised to valdecoxib, the interactions are usually considered to be due to the effects of valdecoxib. The manufacturer of parecoxib cautions the concurrent use with carbamazepine, dexamethasone and rifampicin as their effects on parecoxib have not been studied. Valdecoxib increases the levels of dextromethorphan and omeprazole. Because of these interactions, caution is advised with drugs that are metabolised by the same isoenzymes, nameiy flecainide, metoprolol, propafenone, omeprazoie, diazepam, imipramine and phenytoin. No interaction appears to occur between parecoxib and midazolam.

Inhibitors codeine, encainide, flecainide, fluoxetine, haloperidol, hydrocodone, 4-methoxy-amphetamine, metoprolol, mexiletine, oxycodone, paroxetine, propafenone, propoxyphene, risperidone, selegiline (deprenyl), thioridazine, most tricyclic antidepressants, timolol Fluoxetine, haloperidol, paroxetine, quinidine... 

Concurrent administration of propafenone with digoxin, warfarin, propranolol, or metoprolol increases the serum concentrations of the latter four drugs. Cimetidine slightly increases the propafenone serum concentrations. Additive pharmacological effects can occur when lidocaine, procainamide, and quinidine are combined with propafenone. 

Drug interactions Catecholamine-depleting drugs such as reserpine may have an additive effect in combination with betablockers. Drugs that inhibit CYP2D6 (quinidine, fluoxetine, paroxetine, and propafenone) increase metoprolol concentration.

Genetic differences can also influence the likelihood of some drug interactions. For example, coadministration of the antiarrhythmic propafenone to patients being treated with metoprolol substantially reduces metoprolol metabolic clearance in extensive... 

Propafenone reduces the apparent oral clearance of metoprolol, but it is not clear whether the mechanism is by reduction of true clearance or an increase...
