Memantine interactions

Whereas there arc relatively few iV v/vo studies exploring a possible interaction of memantine with clinically available AChEIs, much more data have been published on the effects of memantine in animals treated with OP or carbamate (CM) insecticides used in agriculture or the household as well as in rodents experimentally intoxicated with the OP nerve ga.s. soman. 

It was suggested that the biochemical mechanism underlying the interaction of memantine with some AChEls could be allosteric modulation of the active center of the enzyme, which in turn could lead to its hindrance and unavailability to AChEls, such as DFP, methyl parathion, and nerve agents (Gupta and Kadel, 1990 McLean et al., 1992 Antonijevic et al, 2002 Stojiljkovic et al, 2002). However, this mechanism remains to be elucidated.

Drugs that make the urine alkaline (e.g. sodium bicarbonate, carbonic anhydrase inhibitors) will reduce the elimination of memantine. Memantine should be used with caution with other NMDA antagonists, such as amantadine, ketamine and dextromethorphan, or concurrent use should be avoided, because of the theoretical increased risk of adverse effects. Memantine is predicted to interact with other drugs eliminated by the same renal secretion mechanism, but no important interaction was seen with glibenclamide, hydrochlorothiazide, metformin or triamterene.
