High expression levels of proteins are potentially attained. In many instances, the level of expression exceeds 1 g protein/litre milk. In one case, initial expression levels of 60 g l-1 were observed, which stabilized at 35 g L1 as lactation continued (the expression of the arantitrypsin gene, under the influence of the ovine P-lactoglobulin promoter, in a transgenic sheep). Even at expression levels of 1 g L1, one transgenic goat would produce a similar quantity of product in 1 day as would be likely recoverable from a 50-1001 bioreactor system.

The patient was developing hepatorenal syndrome with hypematraemia, a coagulopathy and was agitated. The lactate continued to rise to 8, urea 89, creatinine 158 and urine output fell to <30 mL/h. 

CK catalyzes the reversible phosphorylation of creatine in the presence of ATP and magnesium. When creatine phosphate is the substrate, the resulting creatine can be measured as the ninhydrin fluorescent compound, as in the continuous flow Auto Analyzer method. Kinetic methods based on coupled enzymatic reactions are also popular. Tanzer and Gilvarg (40) developed a kinetic method using the two exogenous enzymes pyruvate kinase and lactate dehydrogenase to measure the CK rate by following the oxidation of NADH. In this procedure the main reaction is run in a less favorable direction.

After birth, a rapid drop in progesterone level initially triggers lactation through reversal of inhibitory effects on prolactin (Fig. 44-2). Nipple stimulation then enables lactation to continue once established. During lactation, ineffective removal of milk from the breast, trauma, and skin breaks may lead to problems such as milk stasis, nipple pain, and mastitis.12 Additionally, some women have difficultly initiating lactation due to improper technique and/or activity of inhibitors such as dopamine (Fig. 44-2).

Simultaneous and continuous measurements of extracellular pH, potassium K+, and lactate in an ischemic heart were carried out to study lactic acid production, intracellular acidification, and cellular K+ loss and their quantitative relationships [6, 7], The pH sensor was fabricated on a flexible kapton substrate and the pH sensitive iridium oxide layer was electrodeposited on a planar platinum electrode. Antimony-based pH electrodes have also been used for the measurement of myocardial pH in addition to their application in esophageal acid reflux detection.
