The terminology associated with kidney diseases has been amended and is clarified here. Previously, renal failure was divided into either acute renal failure (ARF) or chronic renal failure (CRF). These terms indicate the rate at which damage occurs rather than the mechanism by which it occurs. Landmark guidelines developed in the USA by the National Kidney Foundation-Kidney Disease Outcomes Quality Initiative (NKF-K/DOQI) attempt to evaluate, classify, and stratify CKD (see Table 45-6). The term renal has largely been replaced by Iddney when referring to chronic disease since it is better understood by patients and nonspecialists. However, acute renal failure (ARF) remains standard nomenclature along with ESRD.

For patients with chronic kidney disease (CKD) the stage of disease should be determined based on the level of kidney function, independent of etiology, according to the National Kidney Foundation Kidney Disease Outcomes Quality Initiative (NKF K/DOQI) CKD classification system.

Since the development of CKD is a complex phenomenon, the Kidney Disease Outcomes Quality Initiative (K/DOQI) has recommended categorizing risk factors associated with CKD as susceptibility, initiation, and progression factors. 

The Kt/Vand urea reduction ratio are the most commonly used indicators for hemodialysis adequacy. The National Kidney Foundation s Kidney Disease Outcomes Quality Initiative has set a goal /Cf/Vof greater than 1.2 and a goal urea reduction ratio of greater than 65%. 

The K/DOQI (Kidney Disease Outcome Quality Initiative) national guidelines recommend a delivered Kt/V of at least 1.2 for a thrice-weekly hemodialysis (NKF-K/DOQI, 2001).
