The most common symptoms are joint pain, reduced range of motion, and brief joint stiffness after periods of inactivity. 

In contrast, the COX-2 enzyme is not produced normally in most tissues, but its production is increased rapidly in the presence of inflammation and local tissue injury. This leads to the synthesis of prostaglandins involved in pain and inflammation. Consequently, blocking the COX-2 enzyme results in analgesic and anti-inflammatory effects. The beneficial effects of NSAIDs in reducing pain, decreasing joint stiffness, and improving function in patients with OA are thought to be due to inhibition of the COX-2 isoenzyme. 

Multiple sulfatase deficiency (MSD). The clinical presentation of MSD includes features of MLD and the MPS, with the features that resemble MLD, such as gait disturbances, psychomotor retardation and polyneuropathy predominating. MPS-like features, such as facial coarsening, hepatosplenomegaly and joint stiffness, are present to a milder and variable extent. A reduction in the activity of all sulfatases is the key diagnostic abnormality. Von Figura and associates have demonstrated that catalytic activity of all sulfatases requires a post-translational modification in which a cysteine residue (cysteine 69 in arylsulfatase A) is oxidized to an aldehyde. It is this modification that is defective in MSD [5]. 

Upon arising, joint stiffness typically lasts less than 30 minutes and resolves with motion. Joint enlargement is related to bony proliferation or to thickening ofthe synovium and joint capsule. The presence of a warm, red, and tender joint may suggest an inflammatory synovitis. 

Joint stiffness typically is worse in the morning, usually exceeds 30 minutes, and may persist all day.

Delayed reactions are defined by the occurrence of arthralgia and joint stiffness (that is a serum sicknesslike reaction) in the days after infliximab administration they have mostly been observed in patients with Crohn s disease who have received episodic treatment.
