To understand the evolution of therapy of the acute coronary syndrome (ACS), which includes unstable angina, acute myocardial infarction, and interventional therapy— percutaneous coronary intervention (PCI), it is most useful to trace the historical events that provided a rationale for the use of anticoagulant and antiplatelet drugs, The focus of this chapter is upon the explosion in knowledge of the physiology of the hemostatic mechanism and will trace the rational development of therapy based upon the pathophysiology of the ACS over the past 40 years.

Preventing misdiagnosis of stroke is increasingly important in the acute stage of the disease when thrombolytic or interventional therapies with potential adverse effects are considered. Misdiagnosis may have serious consequences A misdiagnosed patient may be subject to unjustified thrombolytic therapy and encounter an elevated bleeding risk. Or, another serious nonvascular disorder maybe misclassified as stroke and treatment options maybe missed. 

Apart from surgical and interventional therapy of occlusive carotid artery disease, the major approach to preventing vascular disease and subsequent stroke is to pay close attention to the control of modifiable risk factors such as hypertension, smoking, diabetes, and hypercholesterolemia. Coumadin, an anticoagulant, is effective for the primary and secondary prevention of stroke in patients with atrial fibrillation. Aspirin, clopidogrel, and the combination of aspirin and cUpyridamole have been proven to be effective for secondary stroke prevention along with the antihypertensive combination of indap-amide and perindopril. 

As a rule, the suitability of a combination of cytostatics following operative or local interventional therapy should be discussed at an interdisciplinary conference.
