Urinary incontinence overflow

Patients may or may not be in acute distress. In early stages of disease, the patient may complain of obstructive voiding symptoms. If untreated, in late stages of disease the patient may complain of irritative voiding symptoms, or acute urinary retention, which is painful due to maximal distention of the urinary bladder. Also, the patient may be symptomatic of disease complications, including urosepsis, pyelonephritis, cystitis, or overflow urinary incontinence. 

Explain the pathophysiology of the major types of urinary incontinence (urge, stress, overflow, and functional) and pediatric enuresis. 

In overflow urinary incontinence due to atonic bladder, a trial of bethanecol maybe reasonable if contraindications do not exist. 

In overflow urinary incontinence due to obstruction, the goal of treatment is to relieve the obstruction.

Excessive diuresis and dehydration often cause a transient reduction in glomerular filtration rate and a rise in serum urea (about 8% of all adverse reactions) (SED-8, 350). The sudden diuresis can cause loin pain, particularly in elderly patients, and acute urinary retention and overflow incontinence in elderly men with prostatic hyperplasia.

Urinary incontinence—Involuntary leakage of urine. May result from urethral underactivity (stress urinary incontinence), urethral overactivity (overflow incontinence), or mixed pathophysiologic mechanisms. 

Urinary tract Urinary system adverse reactions, especially enuresis, have been associated with clozapine [SED-15, 832] with an estimated incidence greater than 6%. The mechanisms include overflow incontinence after urinary retention due to the anti-muscarinic action of clozapine or a cholinomimetic activity, and reduced internal urethral sphincter tone caused by aj adrenoceptor blockade enuresis has also been attributed to a non-specific action of clozapine such as excessive sedation, lowering of the seizure threshold, and constipation exacerbating urinary retention and over-fiow. Secondary enuresis in a 21-year man with schizophrenia settled with resolution of his psychotic symptoms but later remerged after starting clozapine [SS ].
