Incidence of infection

The continuous recirculation and spraying results in dirty water building up in the sump. In order to reduce the incidence of infection to occupants and fouling of the nozzle, water treatment with biocides and softening of the water supply are required. The sump is complete with strainers in a position which allows easy access for cleaning. See Fig. 9.15. 

Interferon gamma-lb (Imukin ) is used to reduce the incidence of infections in the treatment of chronic granulomatosis. 

Adverse reactions are common and may include fever (63%), chills (57%), headache (40%), nausea (37%), diarrhea (37%), malaise (13%), dizziness (9%), leukopenia (57%), thrombocytopenia (37%), and generalized pain (46%).7,8,11 The incidence of infection is 37%, with CMV disease occurring in 13% of patients. There are no reported drug-drug interactions (DDIs) with the use of antithymocyte globulin at this time.7,8,11... 

Although study results are conflicting, the daily consumption of active Lactobacillus acidophilus may reduce recurrence. One study found that the daily ingestion of 8 ounces of yogurt produced a threefold reduction in the incidence of infection,8 while other studies found no difference in infection rates in women who ingested yogurt.9,10...

Obesity may also contribute to gallbladder disease, gout, breathing problems, increased incidence of infections, liver diseases, and increased pain, especially in the lower back and knees. 

The use of prophylactic systemic antibiotics in patients with short-term catheterization reduces the incidence of infection over the first 4 to 7 days. 

It is generally accepted that malnutrition decreases the effectiveness of the immune system so that it increases the incidence of infections. Thus famine and infections are always considered together since it is assumed that the former exacerbate the latter. The influenza pandemic in 1918-19 in Europe led to the deaths of more than 20 million people. Poor nutrition caused by the First World War may well have impaired the immune system in many people, thus contributing to the large number of deaths. This topic is discussed in Chapter 18. 

Elderly No overall differences in safety or effectiveness were observed in clinical trials between elderly and younger patients. However, because there is a higher incidence of infections in the elderly population in general, use caution in treating the elderly.

Treatment with interferon gamma has been especially effective in reducing the incidence of infections in patients with CGD (87). The frequency of life-threatening bacterial infections in patients with CGD is reduced by daily prophylactic treatment with trimethoprimsulfamethoxazole (86). 

Interferon gamma is an activator of macrophages. Its anti-viral activity is limited compared to that of interferon alfa. Human recombinant interferon gamma restores, at least in part, macrophage cytotoxicity and with that decreases the incidence of infections in patients with chronic granulomatous diseases. Its adverse effects consist mainly of flu-like syndrome skin rashes may occur. 

Mechanism of Action A colony-stimulating factor that regulates production of neutrophils within bone marrow. Also a glycoprotein that primarily affects neutrophil progenitor proliferation, differentiation, and selected end-cell functional activation. Adverse effects include nephrotoxicity, hypertension, tremor, seizures, increased incidence of infections, gingival hyperplasia, hirsutism, flushing, paraesthesias, tinnitus, headache, gynaecomastia and conjunctivitis. It has no toxic effects on bone marrow and RE system.

B. Indications and nse Neulasta is indicated to decrease the incidence of infection, as manifested by febrile neutropenia, in patients with nonmyeloid malignancies receiving myelosuppressive anticancer drugs associated with a clinically significant incidence of febrile neutropenia. 

Several portable inhalation devices have been developed and are being tested to determine whether they improve protein and peptide delivery via the airways. Aerosolized DNase has been shown in patients with cystic flbrosis to significantly reduce the buildup of mucus in the lung and the incidence of infections. Devices for delivery of therapeutic proteins to deep-lung alveoli to achieve systemic effects are also in development. These products are formulated so that the device aerosolizes the protein in a defined particle size range that cannot be easily achieved by means of conventional metered dose inhalers. 

Several age-related changes contribute to the high incidence of infections in geriatric patients. There appears to be a reduction in host defenses in the elderly, manifested in the increase in both serious infections and cancer. This may reflect an alteration in T-lymphocyte function. In the lungs, a major age and tobacco-dependent decrease in mucociliary clearance significantly increases susceptibility to infection. In the urinary tract, the incidence of serious infection is greatly increased by urinary retention and catheterization in men.
