Documentation of vaccination is critical to an immunization service. In many cases, reimbursement is not obtained without proper documentation. However, with a large portion of immunizations paid for in cash and the need to know which patient received which vaccination in case of a recall, there are some documentation issues that must be addressed. The Centers for Medicare and Medicaid Services has incorporated pneumococcal and influenza immunization rates into some of their quality standards. Patients admitted to a hospital for community-acquired pneumonia should be screened for, offered, and vaccinated with pneumococcal and influenza vaccines prior to discharge if not previously administered. In physicians office practice, all persons over 65 years of age who have been hospitalized in the past year should be screened for, offered, and vaccinated with pneumococcal and influenza vaccines if not previously administered. While the funded mandate would cover all insured persons, the voucher provision would cover uninsured children and adults, who would receive immunizations from health care providers oftheir choice, conditional onthe provider s being willing to accept the vouchers as payment in full. The Certificate of Confidentiality is a federally sponsored document that provides immunity to investigators from any governmental or civil order to disclose identifying information from research records. The Certificate is granted under 301(d) of the Public Healthy Service Act. The Certificate provides additional protection against disclosure of confidential information to outside parties. When a Certificate of Confidentiality is granted, both the protections and limitations of it should be explained to the child and caregiver. Vaccinations can also confer enormous benefits to populations. When vaccination rates are sufficiently high they can create "herd immunity," a condition that occurs when the rate of susceptibility within a population is so low it interrupts the chain of transmission of a disease within a community (Board of Health Care Services 2004). "Herd immunity" is a classic public good it benefits all individuals within a population. [Pg.79]

In health care, our definition of competitors sometimes has to be broadened beyond that which is most obvious. For example, in the hospital environment, a new pharmacy program actually may compete with other professions within the hospital. Many of the advanced clinical services that pharmacists provide replace functions of other health care professionals, especially physicians. This is true even in the outpatient environment. For example, a pharmacy-run immunization clinic may compete with physician offices that also administer vaccines. [Pg.52]
