Heterotopic ossification

Heterotopic ossification (etidronate) Prevention and treatment of heterotopic ossification following total hip replacement or caused by spinal injury. 

Lactation It is not known whether these drugs are excreted in breast milk. Because zoledronic acid binds to bone long-term, do not administer to a nursing woman. Children Safety and efficacy for use in children have not been established. Children have been treated with etidronate at doses recommended for adults, to prevent heterotopic ossifications or soft tissue calcifications. 

Heterotopic ossification caused kg spinal cord infurg PO 20mg/kg/dayfor2 wk then 10 mg/kg/day for 10 wk. 

Heterotopic ossification complicating total hip replacement PO 20 mg/kg/day for 1 mo before surgery then 20 mg g/day for 3 mo after surgery. 

Indomethacin was particularly popular for gout and ankylosing spondylitis. In addition, it has been used to accelerate closure of patent ductus arteriosus. Indomethacin has been tried in numerous small or uncontrolled trials for many other conditions, including Sweet s syndrome, juvenile rheumatoid arthritis, pleurisy, nephrotic syndrome, diabetes insipidus, urticarial vasculitis, postepisiotomy pain, and prophylaxis of heterotopic ossification in arthroplasty.

Inhibition of abnormal calcification initials explorations of BPs as inhibitors of calcification showed promise, and early applications of etidronate included use in myositis ossificans, as well as in patients who had undergone total hip replacement surgery, in order to prevent subsequent heterotopic ossification and to improve mobility [10].

A child with fibrodysplasia ossificans progressiva developed permanent heterotopic ossification at the injection site after intramuscular DTP immunization (80). The authors considered that intramuscular injections in children with fibrodysplasia ossificans progressiva were potentially risky, but that subcutaneous injections could be carried out.

Etidronate is a bisphosphonate that inhibits normal and abnormal bone resorption and reduces bone formation. It is indicated in the treatment of symptomatic Paget s disease prevention and treatment of heterotopic ossification following total hip replacement or caused by spinal cord injury.

Ultrasound will show the soft tissue swelling and thickening of the overlying soft issue and tendon. MR imaging will show fragmentation of the tibial tubercle, heterotopic ossification within the distal patellar tendon and non-specific oedema with a reactive bursitis.

In addition to the inflammatory response observed in periprosthetic tissues, other morphologic changes can take place in tissue and bone surrounding the implant. These changes include tissue fibrosis, necrosis, fibrocartilage formation, heterotopic ossification, and last, but certainly not least, osteolysis of the surrounding bone (Figure 23.6).
