In 1979-1980, some illegal fentanyl analogues appeared that were being sold as substitutes for heroin on the street. Suddenly, a series of more than a dozen mysterious deaths occurred in southern California. Upon autopsy, the victims strongly looked as if they had overdosed on heroin however, no traces of heroin could be found in their bodies. Later, forensic chemists identified a fentanyl analogue (alpha-methyl-fentanyl) that was present in all of the victims. As it turns out, alpha-methyl-fentanyl was being sold on the streets under the name China White (Figure 7.2), because it resembled (and contained) pure synthetic heroin that was produced in Southeast Asia. [Pg.75]

Another problem for the makers of naltrexone was recently uncovered by researchers testing the drug on marijuana smokers. To the researchers surprise, people who were given naltrexone and then smoked marijuana reported that they felt greater psychotropic effects from the marijuana than if they had simply smoked the marijuana alone. In other words, while naltrexone blocks the psychotropic effects of alcohol, heroin and opium, it appears to increase the effects of marijuana. [Pg.8]

Even though the pharmacotherapy of adolescent addictions is a relatively new field, it appears to have a role in the treatment of many youth with SUD. Pharmacological agents have been shown to reduce excessive craving, to reduce associated comorbid psychopathology, and to prevent the future development of SUD in certain at-risk youth. Despite the present lack of empirical evidence, opiate substitution therapy should be considered in youth with heroin addiction. As with all treatments, the expectations, risks, and benefits should be reviewed with the patient and their caretakers. For SUD treatment to be successful, the provider should closely monitor and address the patient s SUD and psychiatric symptoms, treatment compliance, and social stressors. [Pg.614]

Reviewing her history, it appeared that she had used heroin by injection for five years before being first prescribed methadone. She was from a relatively isolated small town where the limited network of users had few links with treatment services.
