Hepatobiliary agents

Technetium-99m mebrofenin is an iminodiacetic acid derivative used as a hepatobiliary agent. The kit is suppHed as a single vial containing lyopbilized mebrofenin. The reconstituted kit has 18-hour usage, owing to the preservative, propylparaben. 

The hepatobiliary agents are removed from the blood by hepatocytes in the liver through an active transport mechanism.677 They are cleared from the plasma as anions by the same general mechanism as bilirubin so that high bilirubin levels can competitively inhibit the transport of "mTc-IDA agents through the liver.553... 

Intestinal absorption studies of Mn-MP were undertaken in an effort to assess the viability of the metalloporphyrin as an oral hepatobiliary agent [101, 102]. Mixed micelles of Mn-MP complexed with monoolein and taurocholate were administered to rats, resulting in liver image enhancement 68% above baseline levels six hours after administration [101]. In pigs, the mixed micelle preparation showed variable enhancement over 24 hours. Observation that Mn-MP interacts with oleic acid vesicles [103] led to investigations of the effect of oleic acid on the absorption rate of Mn-MP from the small bowel into the circulatory system [102,104]. The increase in absorption of the complex was mediated by a decrease in the relaxivity of the metalloporphyrin resulting from the interaction with the lipid vesicles.

AT-(2,6-dimethylphenylcarbamoylmethyl)iminodiacetic acid (HIDA) and several ether derivatives have been evaluated as ligands for complexation, producing Tc complexes suitable as hepatobiliary agents. Tc-IDA complexes have a negative charge (Loberg and Fields 1978). Two molecules of ligand are coordinated to one Tc(III)-core (Nunn et al. 1983) (Fig. 2.1.17).
