Glucosamine sulfate

Chitosan freeze-dried fleeces support chondrocyte attachment and synthesis of extracellular matrix [344]. Chitosan was used to assist the spontaneous tissue repair of the meniscus [345]. The repair of the cartilage and the prevention of its degradation in osteoarthritis is, however, possible with the association of glucosamine sulfate salt and chondroitine sulfate, the latter being particularly effective [346].

Because of their relative safely, a trial of glucosamine-chondroitin may be reasonable in patients considering alternatives to traditional OA treatment. Dosing should be at least glucosamine sulfate 1,500 mg/day and chondroitin sulfate 1,200 mg/day in divided doses.

Glucosamine sulfate is very well tolerated. In clinical trials, mild diarrhea and nausea were occasionally reported. Cross allergenicity in people with shellfish allergies is a potential concern however, this is unlikely if the formulation has been properly manufactured and purified. 

Glucosamine sulfate may increase the International Normalized Ratio (INR) in patients taking warfarin, increasing the risk for bruising and bleeding. The mechanism is not well understood and may be dose-related as increases in INR have occurred when the glucosamine dose was increased. Until more is known, the combination should be avoided or very carefully monitored.

Pharmacokinetic studies on glucosamine sulfate in man showed that uptake into plasma proteins peaked at 8-10 hr, and the half-life was about 70 hr after administration. However, oral administration yielded only 26% of the bioavailability (measured from plasma levels after any first pass metabolism) of the other two routes of administration via injection, but the glucosamine sulfate was detected in bones and cartilage afterwards. ...

Osteoarthritis is treated with glucosamine sulfate which is a precursor of glycosamino-glycan in joint cartilage (Parcell 2002). Glucosamine sulfate enhances the sulfate content in the synovia and blood serum (Hoffer et al. 2001). In contrast, glycosami-noglycan synthesis is reduced in sulfur deficiency (van der Kraan et al. 1990). 

In contrast, a 71-year-old man stabilised on acenocoumarol 15 mg weekly had a decrease in his INR to 1.6 after taking glucosamine sulfate (Xicil) 1.5 g daily for 10 days. The glucosamine was stopped and the INR reached 2.1. When the glucosamine was restarted, with an increase in acenocoumarol dose to 17 mg weekly, the INR only reached 1.9.
