Proteinuria occurs in about 7% of patients using bucillamine (1). Less often, nephrotic sjmdrome can develop (5-9). Microscopically there is membranous glomerulonephritis, with diffuse fine granular precipitation of IgG and C3 around the capillary walls, electron-dense deposits at the subepithehal side of the basement membrane and effacement of foot processes. 

Glomerulonephritis—Glomerular lesions characterized by inflammation of the capfilary loops in the glomerulus caused by immunologic, vascular, and other idiopathic diseases (may be diffuse or mem-branoprolrferative).

Cryoglobulins are special immunoglobulins that reversibly precipitate from serum at cold temperatures. Cryoglobulins can be classified as monoclonal globulins (type I), mix of polyclonal-monoclonal (type II), or mix of polyclonal-polyclonal (type III) immunoglobulins. Cryoglobulins can precipitate in different tissues of the body such as the kidney and the extremities. They are associated with several immune-type disorders, viral infection, glomerulonephritis, peripheral neuropathy, and diffuse vasculitis. 

Urinary tract A case report was published on anti-neutrophil cytoplasmic antibody (ANCA)-positive pauci-immxme glomerulonephritis during febuxostat treatment. A 63-year-old African started treatment with febuxostat (dose not reported) because he had a history of chronic asymptomatic hyperuricemia which was not adequately controlled by allopurinol therapy. After 6 months of febuxostat therapy he developed AKI, characterised by acute renal failure, nephritic syndrome, proteinuria, microscopic haematuria and aseptic leukocyturia. Within 48 h after discontinuation of febuxostat, serum creatinine levels improved, although nephritic syndrome remained. ANCA-positive pauci-immune glomerulonephritis was confirmed by renal biopsy revealing diffuse crescentic necrotizing glomerulonephritis [60 ].
