As a part of the power demonstration program of the AFC in the 1950s, the Enrico Fermi fast breeder reactor (Fermi-1) was built near Detroit by a consortium of companies led by Detroit Edison. Fermi-1 used enriched uranium as fuel and sodium as coolant, and produced 61 MWe. It suffered a partial fuel melting accident in 1966 as the result of a blockage of core coolant flow by a metal plate. The reactor was repaired but shut down permanently in November 1972 because of lack of binding. Valuable experience was gained from its operation, however (58). 

In most cases volume requirement is the primary consideration for selecting the most cost-effective process. However, in many cases a lower volume process may be used during initial phases of a program and then be replaced by a higher volume process when the product has gained field acceptance. 

Buy-in" of facility management. Cooperation of facility-based staff and managers strongly influences the effectiveness of baseline PSM assessments. The assessment phase may be the first visible manifestation of your company s PSM initiative at the local level, and should be properly understood by those participating—especially those whose PSM activities and programs are under review. An assessment method with which local personnel are at least somewhat familiar, conducted by professionals whose skills they respect, will more likely gain buy-in and cooperation. 

Following the implementation of the error management program, evaluations of its effectiveness must be made on a continuing basis. Measures of effectiveness can be gained partly by direct discussions with individuals at all levels of the plant, using a predesigned evaluation checklist to ensure that all the evaluation dimensions are assessed in a systematic way.
