As noted earlier, an accident investigation is not eomplete until a report is prepared and submitted to proper authorities. Special report forms are available in many

Other instances may require a more extended report. Such reports are often very elaborate and may include a cover page, a title page, an abstract, a table of contents, a commentary or narrative portion, a discussion of probable causes, and a section on conclusions and recommendations. The following outline has been found especially useful in developing the infoimation to be included in the formal report

One final report of alkane activation has been reported by Moiseev. The mechanism of the reaction was not investigated, but this system might be classified as an electrophilic activation of methane, either of the Shilov type or of the concerted four-center type (Fig. lc) where X=triflate. Reaction of methane with cobalt(III)triflate in triflic acid solution leads to the formation of methyltriflate in nearly stoichiometric quantities (90% based on Co) (Eq. 18). Carbon dioxide was also observed, but not quantified. Addition of 02 led to catalysis (four turnovers) [79]. 

Principal Investigator, as in the case of a subcontracted study, or to another Responsible Scientist , as for the preparation and assessment of histopathological slides in the specialised laboratory within his test facility, but the ultimate responsibility of the Study Director as the single central point of control cannot be delegated. The Study Director has finally to acknowledge this by signing the GLP Statement in the final report of the study he has directed. 

The Principles of GLP require Quality Assurance to prepare and sign a statement, to be included with the final report, which specifies types of inspections and their dates, including the phase(s) of the study inspected, and the dates inspection results were reported to management and the Study Director and Principal Investigator(s), if applicable . Thus, after having audited the final report of the study, and after having been satisfied with the way the raw data have been recorded and represented in the final report. Quality Assurance has to look at its own records to identify the respective activities performed in connection with the study in question. Quality Assurance has then to draw up a list of such activities with their dates and to include this list in the Quality Assurance statement.

The crew attempted to fly the aircraft in accordance with procedures they had practised many times in the DC-10 simulator. Unfortunately, these procedures called for a speed that was too slow, and the aircraft stalled and crashed.
