Fever may occur after stroke for a number of reasons (Box 20.3). Animal studies and observational data in humans suggest that pyrexia increases infarct size and is associated with poor outcome, and that the converse is true for hypothermia, but risks and benefits of active cooling in acute stroke are yet to be determined (Reith et al. 1996 Hemmen and Lyden 2007 Sacco et al. 2007). Infection should be treated promptly but there is no evidence to support the routine use of antipyretics such as paracetamol for temperatures above 37.5 °C, although they are widely used in practice. [Pg.254]

Nine of 31 Japanese children receiving maintenance vincristine chemotherapy for either leukemia or lymphoma developed a fever over 38°C (83). An allergic mechanism was proposed. Concurrent glucocorticoids as part of the regimen appeared to be protective. Younger children were at greater risk. Peak temperatures occurred within 24 hours of vincristine administration. The pyrexia lasted from 6 hours to 4 days and the condition was self-limiting. [Pg.3637]

Toxicity Adverse effects include headache, malaise, weakness, and anorexia. Reactions to proteins released by dying filariae include fever, rashes, ocular damage, joint and muscle pain, and lymphangitis. Observational stndies In a multicenter, open study of a pediatric oral famciclovir formulation in 18 infants aged 1-12 months with a suspicion or evidence of herpes simplex virus infection, 8 had at least 1 adverse event, aU within 8 days, and most after 3 or more days, with the exception of 1 case of fever reported after 31 days [22. Of 17 individual adverse events recorded, the most common were vomiting (n = 3), diarrhea (n=2), pyrexia (n = 2) and dehydration (n=2). The only severe adverse event was in an immimo-compromised infant who had oral candidiasis and developed worsening herpes simplex virus encephalitis, but this was not thought to have been related to famciclovir. [Pg.451]
