External reporting systems

These reports were in addition to the internal reporting for OSHA PSM and various local or state requirements. While Quality Chemical had long ago consolidated a number of these programs, there were still four different accident/incident reporting systems—one for external reporting of releases, one for external reporting of injuries and other nonrelease events, one for a corporate-wide database and one for each site itself.

Aeronautics and Space Administration (NASA) for the aviation industry. NASA, external to the industry, accepts confldential narratives about near misses and accidents up to the point of a crash (Reynard, Billings, Cheaney, and Hardy, 1986). The system is voluntary, confidential, and nonpunitive, and its purpose is to collect and use incident data to improve the national aviation system. The ASRS supports aviation system policy, planning, and improvement and strengthens the foundation of human factors research in aviation by identifying deficiencies for correction by appropriate authorities. The Veterans Administration is the only health care entity to contract with NASA for a blameless reporting system at this time. 

Figure 1 shows a number of possible planar array geometries. The array elements in the figure may be amperometric, potentiometric, or impedimetric working electrodes, and each element may consist of a complete electrochemical cell. The most widely reported devices contain metallic amperometric or voltammetric working electrode arrays that employ common counter and reference electrodes that may be coplanar or external. Reported element dimensions vary from the mm scale down to tens of nm. Many reports describe arrays that have been encapsulated into microfluidic chips, and some have specialized external electronic and software systems to allow multiplexed sequential or simultaneous data acquisition from aU elements.
