Evidence of failure

Viewed in perspective, evidence of failure in service has been rare and the practical hazard is certainly very much lower than would appear from the results of laboratory tests. In chlorinated hydrocarbons the effect can be controlled by the addition of inhibitors, and, for example, the appropriate commercial degreasants containing these inhibitors are specified in a British detence standard. ... 

The above tests for characterising coating properties necessarily continue to involve a certain amount of empiricism. The intelligent use of these tests, however, has shown that wide variations of physical and electrochemical characteristics of coatings as a function of composition may be obtained, and further, that significant changes in these characteristics, that can be measured before the usual evidence of failure appears, occur upon natural and accelerated ageing. 

Infants and children with maldigestion may have evidence of failure to thrive (below age-based normals in both height and weight), and adults may be near or below their ideal body weight despite apparent adequate caloric intake.

Act or a regulation is alleged, the fact that the code was not followed will be accepted in court as evidence of failure to do all that was reasonably practicable. A defence would be to prove that something equally as good or better had been done (s. 17(2)). To supplement the Approved Codes of Practice, the Executive issue guidance booklets and notes which are purely advisory and have no standing in law but do indicate means for complying with statutory requirements.

In the meantime, new designs of lithium batteries were developed, tested, and evaluated, and newly developed batteries demonstrated significant improvement in battery performance parameters, including energy density, shelf life, longevity, reliability, and consistent electrical performance under high and low operating temperatures with no evidence of failure mechanisms. 

Until recently, the only measuring tools available were the counting of failures (lost-time injuries however defined, first-aid cases, property damage incidents and near misses ), and attempts at measuring the financial costs of losses resulting from failures to control safety, health and the environment. All of these involve studying the evidence of failures in one form or another, rather than the performance achieved. And there are difficulties in collecting the evidence, for example ... 

In any case, comparisons with data show no evidence of failure of the linear model at low electric fields. In fact, the Unear model arguably Uts the low electric field dafa beffer than the = T" + fiTp)" models.
