Typically, for any given pressure, industrial packaged boilers operate at higher heat-flux rates than field-erected boilers, This requires that the package boiler FW quality should be substantially better (i.e., lower overall TDS and lower levels of silica and sodium). Appropriate MU water pretreatment may, for example, necessitate the use of twin bed and mixed bed demineralization ion exchange, or RO and mixed bed (in addition to mechanical deaeration and other processes). 

Historically, the first effective pharmacologic agents for lowering the blood pressure were the ganglionic blockers. At the level of the ganglia, these compounds block both sympathetic and parasympathetic transmission. The decrease in parasympathetic function is responsible for urinary retention, for the failure to develop an erection in the male patient and for the paralytic ileus. 

At clinical trials, sildenafil did not work well as a treatment for angina. Instead, it was observed that it overcomes erectile dysfunction. Later, it was found that cyclic GMP also increases the level of nitric oxide, which is needed in penile erections.

A further characteristic of this principle is that, if the activity of phosphodiesterase is decreased, the concentration of cyclic GMP will increase to an extent dependent upon the extent of the decrease in activity. This characteristic has been made use of by the pharmaceutical industry. Cyclic GMP has a vasodilatory effect and this is the case for the arterioles that supply blood to the corpus cavemosum in the penis, which controls the erection of the penis. Drugs were developed (e.g. sildenafil) that inhibits cyclic GMP phosphodiesterase and hence increases the cyclic GMP level which resnlts in vasodilation of the arterioles and an increase in the snpply of blood to the spongy tissue of the corpus cavemosum, which expands resulting in erection. This dmg has been found to be effective in some patients snffering from erectile dysfunction. This can be a particular problem in diabetic patients and more elderly men (Chapter 19). 

Sildenafil is readily absorbed after oral administration and reaches peak plasma levels after about an hour. It undergoes hepatic metabolism and has a terminal half-life of about 4 hours. An initial dose of 50 mg is taken about an hour prior to sexual activity to induce penile erection.

With many polymerizations reactor cleaning is very important. On large reactors manual cleaning of the inside walls is less economical than mechanical or chemical cleaning. In manual wall cleaning with scraper tools a scaffold must be erected in the reactor, or a raft must be introduced to float on water, on which the workmen can stand and lower the level from which to work by draining water away. 

Sildenafil increases the release of nitric oxide and increases the levels of cyclic guanosine monophosphate (cGMP), a smooth muscle relaxant. Sildenafil enhances the effects of nitric oxide by inhibiting phosphodiesterase 5, an enzyme found primarily in the penis that degrades cGMP. As a result, increased levels of cGMP in the corpus cavemosum enhance smooth muscle relaxation, the inflow of blood, and erection. Sildenafil has no effect in the absence of sexual stimulation (see Figure 60.1). 

Hormonal therapy with testosterone should be reserved primarily for patients with hypogonadal disorders. There are two important warnings about the indiscriminate use of intramuscular testosterone in patients with serum testosterone levels in the normal range. First, many impotent patients are older and may have adenocarcinoma of the prostate, thus exogenous testosterone may accelerate the growth of the neoplasm. Second, although testosterone may induce a marked increase in libido, patients may still be unable to achieve adequate erection.
