Eczema pompholyx

However, one is bound to wonder whether this was a direct reaction to levonorgestrel or a stress reaction to the absence of menstrual periods. Like many other users of Mirena, this woman had amenorrhea associated with facial flushing and pustules conditions such as urticaria, eczema, pompholyx, and erythema multiforme occur cyclically in some women in the second half of the menstrual cycle, irrespective of contraceptive use. 

Skin Cutaneous reactions associated with intravenous immunoglobulin are uncommon, but include pruritus, non-specific eruptions, erythema, urticaria, eczema, pompholyx, pete-chiae, and skin hemorrhage [53, 54 , 55 , 57 , 59 ]. 

Pompholyx eczema is a special vesicular type of dermatitis affecting the hands and feet. It can be acute and persistent, characterised by many deep-seated, itchy, clear, tiny blisters. Later there may be scaling, fissures and thickening of the skin. Outbreaks usually last several weeks, and common sites include sides of the fingers, palms and, less often, on the soles. The aim of treatment is to prevent secondary infection and spontaneous resolution is expected within 2 or 3 weeks. If, however, it persists, it may be necessary to use short courses of corticosteroid creams. It is an inflammatory reaction and there is no underlying infective component. 

Systemic allergic reactions have been reported in eight patients after topical application of a wart formulation containing resorcinol (3). All developed a marked eczematous, sometimes bullous reaction, localized to the site of application in four cases there were generalized urticaria and angioedema, in one pompholyx eczema, and in three generalized eczema with pompholyx. In aU cases there were positive patch tests with resorcinol. 

The most common form of tinea pedis is located in the toe webs. The fourth interdigital web is preferentially infected, but the other webs can also be infected, on one or both feet. The clinical symptoms are characterised by a whitish, diffuse maceration in the cleft marginated by a collarette of continuous desquamation (Fig. 10). In some cases, there is a small, painful fissure running along the line of the cleft. The entire area is ulcerative and macerated from microbial superinfection. Itching is usually present. In T. rubrum infections, a squamous, hyperkeratotic variety that is particularly chronic and resistant to treatment and that affects the soles, heels and sides of the feet ( moccasin foot ), is often found (Hay and Moore 1998). The dorsal surfaces of the toes and feet are not often affected, but associated onychomycosis is common. Tinea pedis is sometimes associated with reactive plantar pompholyx extremely pruritic, coalescent vesicular eczema is observed. Microscopic examination sometimes reveals the presence of dermatophytic filaments in these so-called id reactions. 

Pompholyx or dyshidrotic-like eczema due to rubber allergies is found in bank employees and other workers with frequent exposure to rubber bands. The dermatitis principally affects the dorsolateral aspects of the digits of the dominant hand. The chief sensitizers are thiurams and thiazoles (Conde-Salazar 1990b). 

Of 527 workers in a poultry slaughterhouse, 343 (65.1%) had skin diseases. This included 283 of 386 (73%) who worked in direct contact with the poultry, compared with 2 of 39 (5.1%) office workers or supervisors in the same industry. The most common skin problems were maceration and interdigital erosion, paronychion, pompholyx, and eczema and onychomycosis (Hayashi et al. 1989). The dermatoses were caused mainly by the wet work, and a marked reduction in number of dermatoses was seen after implementation of a program to protect the hands.
