Dyspepsia nonspecific

Antiflatulents are used for die relief of painful symptoms of excess gas in the digestive tract. These drugs are useful as adjunctive treatment of any condition in which gas retention may be a problem (ie, postoperative gaseous distention, air swallowing, dyspepsia, peptic ulcer, irritable colon, or diverticulosis). In addition to its use for tiie relief of intestinal gas, charcoal may be used in the prevention of nonspecific pruritus associated with kidney dialysis treatment and as an antidote in poisoning. Simethicone is in some antacid products, such as Mylanta liquid and Di-Gel liquid. 

Adverse reactions occurring in 3% or more of patients include abdominal pain, accidents, chest pain (nonspecific), constipation, dry mouth, dyspepsia, edema (dependent), epistaxis, infection, moniliasis, myalgia, pharyngitis, rash, rhinitis, sinusitis, upper respiratory tract infection, urinary tract infection, vomiting. 

Rofecoxib is approved for the treatment of osteoarthritis, dysmenorrhea, and acute pain. The most common adverse reactions to rofecoxib are mUd to moderate GI irritation (diarrhea, nausea, vomiting, dyspepsia, abdominal pain). Lower extremity edema and hypertension occur relatively frequently (about 3.5%). It is not metabohzed by CYP2C9, so rofecoxib should not be subject to some of the interactions seen with celecoxib. However, its metabolism is increased by the coadministration of rifampin, which acts as a nonspecific inducer of hepatic metabolism.

In spite of the lack of comparative trials, nonprescription bismuth compounds are widely used by patients for the nonspecific treatment of dyspepsia and acute diarrhea. Bismuth subsalicylate also is used for the prevention of traveler s diarrhea (30 ml or 2 tablets four times daily). 

Patients with early ovarian cancer can present with nonspecific, vague abdominal symptoms such as nausea, discomfort, dyspepsia, flatulence, bloating, fullness, early satiety, and digestive disturbances. " These symptoms can easily be confused with symptoms that happen normally throughout the menstrual cycle. Late symptoms can include pain, abdominal distention, ascites, and abdominal or pelvic masses. " A palpable ovary in a postmenopausal woman should be promptly evaluated because functional cysts do not usually occur in this age group. ... 

Clinical manifestations of brucellosis are diverse and the course of the disease is variable.32 Patients with brucellosis may present with an acute, systemic febrile illness an insidious chronic infection or a localized inflammatory process. Disease may be abrupt or insidious in onset, with an incubation period of 3 days to several weeks. Patients usually complain of nonspecific symptoms such as fever, sweats, fatigue, anorexia, and muscle or joint aches (Table 25-2). Neuropsychiatric symptoms, notably depression, headache, and irritability, occur frequently. In addition, focal infection of bone, joints, or genitourinary tract may cause local pain.
