Solutions are used topieally as well as internally. Examples of eompounded liquids include topicals (wart solutions), oral syrups and elixirs, nasal solutions, otic solutions, iontophoretic solutions (dexamethasone sodium phosphate), and many others (Table 5).

Liposomes have been used in the local delivery of drugs to the oral mucosa. Farshi et al. [96] studied the biodistribution of dexamethasone sodium phosphate (DSP) encapsulated in multilamellar vesicle (MLV) liposomes labeled with 99mTc in ulcerated and intact oral mucosae of rats. The liposomes were found to localize the drug in the ulcerated area and increase local drug concentration while decreasing systemic concentration.

For parenteral administration in intensive therapy or in emergendes, the sodium phosphate ester may be given intravenously by injection or infusion, or intramuscularly by injection. Dexamethasone sodium phosphate is also used in the treatment of cerebral edema caused by malignancy. The sodium phosphate ester is given by intra-articular, intra-lesional or soft tissue injection. Dexamethasone acetate may be given by intramuscular injection in conditions where corticosteroid treatment is indicated, but a prompt response of short duration is not required.

A masked study using male volimteers compared ocular pressure elevations with dexamethasone phosphate 0.1%, fluorometholone alcohol 0.1%, and medrysone 1% applied four times daily for 6 weeks. Figure 12-3 shows the relative ability of these steroids to raise lOP. At the end of 6 weeks of treatment, the mean pressure increases fiar dexamethasone, fluorometholone, and medrysone were 63.1%, 33.8%, and 8.3%, respectively. Additional studies have compared the effects of fluorometholone alcohol suspension 0.25% with dexamethasone sodium phosphate solution 0.1% in steroid-responsive patients. Subjects received the medication in one eye four times daily for up to 6 weeks. Although both drugs elevated lOP, mean pressure increases from baseline in eyes treated with fluorometholone were significantly lower than those in eyes treated with dexamethasone at weeks 2,4, and 6. Further studies are needed to compare the effects of the alcohol and acetate derivatives of fluorometholone on lOP in both nonsteroid and steroid responders. 

Topical or periocular steroids cause few systemic effects. When topical dexamethasone sodium phosphate was administered four times daily for 6 weeks, subjects showed reduced plasma levels of cortisol. However, elevation of 11-deoxycortisol with the oral metyrapone tartrate test indicated that the pituitary-adrenal axis was intact.

A typical example of this type of activation-controlled CrDDS is the development of an iontophore-tic drug delivery system, named Phoresor by Motion Control, to facilitate the percutaneous penetration of antiinflammatory drugs, such as dexamethasone sodium phosphate, to surface tissues. 

Dexamethasone acetate (Dalalone DP, Forest, Decadron-LA, Merck) and Dexamethasone sodium phosphate (Merck) are available as a suspension or a solution. These dexamethasone formulations contain creatine or creatinine as additives. 

Itraconazole markedly increases both systemic exposure to dexamethasone and its effects. This interaction has been investigated in a randomized, double-blind, placebo-controlled, crossover study (94). Eight healthy volunteers took either oral itraconazole 200 mg od or placebo for 4 days. On day 4, each subject was given oral dexamethasone 4.5 mg or intravenous dexamethasone sodium phosphate 5.0 mg. Itraconazole reduced the systemic clearance of intravenous dexamethasone by 68%, and increased its AUC and prolonged its half-life more than three-fold the AUC of oral dexamethasone was increased nearly four-fold and its half-life nearly three-fold. Morning plasma cortisol concentrations at 47 and 71 hours after dexamethasone were significantly lower after itraconazole than placebo.
