Deoxycortone

Are tablets use for sustained action and implanted under the skin e.g. Deoxycortone acetone (for contraception). 

Water retention occasionally occurs and may reflect a degree of deoxycortone acetate (DOCA)-like activity virilization of a female fetus is more likely to occur with a product that has some androgenic activity, and breast tenderness with a product that has estrogenic activity. 

Thus, in patients with Addison s disease or other forms of adrenal insufficiency, continuing oral administration of cortisone acetate or fludrocortisone acetate enables salt balance to be restored. Other corticosteriods and analogues that have been used in the hormonal control of sodium levels include aldosterone and deoxycortone acetate. Individual corticosteroids vary in the extent to which they possess the various hormonal activities so that combination therapy is usually required if, for example, mineral balances are to be maintained when corticosteroids are administered for their anti-inflammatory, antirheumatic or anti-allergic properties.

Infra-red Spectrum. Principal peaks at wavenumbers 1242,1667, 1744, 1718, 1203, 1610 (deoxycortone acetate, KBr disk). 

Dose. Usually 1 to 5 mg of deoxycortone acetate daily, intramuscularly sublingual doses of 2 to 10 mg daily have also been given.

Tang et al. [58] reported the analysis of corticosteroids in eqnine urine after enzymatic hydrolysis, liquid-liquid extraction (LLE) with ethyl acetate, and LC-MS-MS in positive-ion APCl mode. Hydrocortisone, deoxycorticosterone, and 21 synthetic corticosteroids can be analysed at 5 pg/1 level in urine within 10 min. As an example, the confirmation of deoxycortone in a urine sample is demonstrated in Figure 13.8.

Systemic corticosteroids can increase the loss of potassium, particularly those that are naturally occurring (cortisone, deoxycortone, hydrocortisone) whereas the synthetic derivatives (betamethasone, dexametha-sone, methylprednisolone, prednisolone, prednisone, triamcinolone) have much less mineralocorticoid activity. There is therefore the possibility of potassium depletion, particularly when corticosteroids are used long-term, which may increase the risk of digitalis toxicity. These corticosteroids also cause sodium and water retention, resulting in oedema and hypertension, which can lead to cardiac failure in some individuals.
