The occurence of drug interactions involving clofazimine have also been investigated. Most of the studies show that clofazimine does not exert any effect on dapsone excretion in leprosy patients (Balakrishnan and Seshadri, 1981 Zuidema et al., 1986). Clofazimine has been shown to significantly reduce the absorption of simultaneously administered rifampicin, resulting in delayed time to reach peak serum concentration and increased t j. No significant changes were seen in Cmax or AUC (Mehta et al., 1986).

Enfuvirtide had no effect on the metabolism of dapsone or de-brisoquine, and had little effect on the metabolism of caffeine, chlorzoxazone, and mephenytoin. Thus, it is not anticipated that enfuvirtide would cause clinically important drug interactions with drugs metabolised by the cytochrome P4S0 isoenzymes.

Buffering agents that are compounded with didanosine to counteract its degradation by gastric acid may interfere with the absorption of other drugs that require acidity (e.g., indinavir, delavirdine, ketoconazole, fluoroquinolones, tetracyclines, dapsone). An enteric-coated formulation Videx EC) that dissolves in the basic pH of the small intestine is not susceptible to these interactions. Ganciclovir and valganciclovir can increase blood levels of didanosine. The use of zalcitabine with didanosine is not recommended because that combination carries an additive risk of peripheral neuropathy. Critical to the success of this screening approach is the selection and use of probe substrates that have a specific metabolic pathway mediated by a single CYP. An example of specific CYP probe substrates initially studied by Dear and co-workers (Ayrton et al., 1998a) is shown in Table 6.9. Many laboratories conduct detailed validation of CYP probe substrates to determine whether they can be simultaneously administered without metabolic interaction. One metabolic cocktail consists of the probe drugs caffeine, chlorzoxazone, dapsone, debrisoquin, and mephenytoin and is referred to as the Pittsburgh cocktail (Frye et al., 1997). Interactions. Rifampicin is a powerful enzyme inducer and speeds the metabolism of numerous drugs, including warfarin, steroid contraceptives, narcotic analgesics, oral antidiabetic agents, phenytoin and dapsone. Appropriate increase in dosage, and alternative methods of contraception, are required to compensate for increased drug metabolism (see also paracetamol overdose, p. 287).
