Gathering data cultural

The first area focuses on the cultural and organizational factors that will have a major influence on the effectiveness of a human error data collection system and how well the information derived from such a system is translated into successful error reduction strategies. Regardless of how effectively the technical issues are dealt with, the system will not be successful imless there is a culture in the organization which provides support for the data gathering process. No data collection system aimed at identifying human error causes of accidents will be workable without the active cooperation of the workforce. 

Due to the pioneering efforts of Gunther, et al, Cl) we now have a standard procedure to determine dislodgeable residues. This is the component of the residue which is considered available for exposure during harvesting and other cultural activities that involve intimate contact with treated foliage. The technique has been widely used and allows the comparison of residue decay dynamics for different compounds under different situations. The data gathered in this way have relevance to potential exposure. 

Meanwhile the matter was being approached from an altogether different angle in New York, by the Wassons, husband and wife, who had spent more than two decades gathering data on the role of mushrooms in primitive societies in Eurasia. This theme in anthropology, which we called ethnomycology, had never before been explored in the West. Eurasia embraced so many cultures and so much history and literature that we had resolved early in our inquiries to stop with Eurasia and leave Africa and the Americas to others. 

One means of gathering data for monitor indicators is patient safety culture questionnaires. We have developed a patient safety culture questionnaire, TUKU, that measures employees perceptions of the organisational functions depicted in Table 9.4 as well as employees psychological states, such as sense of control and worry about patient safety (Reiman et al. 2013). In one hospital, the results of the questionnaire were compared with the ratio of patient safety incidents at the hospital s 40 units 16 months after the safety culture questionnaire was administered. The results, which must be treated with caution due to the small sample size, indicated that perceptions of work process management , work conditions management . 

Now, the data from horizontal and vertical gathering should be combined. The horizontal, high-level view of the safety culture is very useful to frame the safety health of the organization. But the vertical data added to the horizontal view will give real color to the analysis.
