Crisis planning

Of course the fourth aspect to the tetrahedron needs to be considered - the chaui of system reactions and sequence of responses to an accident once initiated and in progress. This aspect needs to deal with the accident once it is under way, to reduce the duration or severity, rather than to prevent it occurring in the first place. There are several strategies in place for this already in industry, perhaps under the name of crisis planning or major accident planning. For example ...

Crisis plans should be proactive and preventative. However, the majority of U.S. schools are still relatively reactive in response to a crisis. During a crisis, school staff members are equally affected by the crisis, making it extremely difficult to organize an effective crisis intervention response. Therefore, research emphasizes the need for schools to put more efforts into planning and prevention of school crises (Pitcher and Poland, 1992 Brock et al., 2001).

Consider writing the patient a bullet-point list of the key discharge / crisis plan (p315) points. This may help them remember appointments and essential messages while awaiting their copy of the discharge summary. They may also need ... 

Depending on your concerns, organise follow-up within the next 7 days (GP/CMHT/HTT). Plan any medication management, e.g. give TTOs to Sally/HTT HTT to prescribe. Write a crisis plan (p315) with Sally and share this with carers. Document your plan and risk... 

Work closely with the receiving CMHT, to make Mike s discharge as smooth as possible. Ensure he has TTOs, a 7-day follow-up with a specific person, and a written crisis plan. The CC should update the VLO, if involved.

If discharging, write Ben a simple crisis plan (p315) for feeling overwhelmed or suicidal, including contact details of carers, his CMHT and any crisis lines (e.g. Samaritans 08457 909090). He should attend ED if worse - explain he ll be taken seriously, even without self-harming.
