Although most work to date has focused on the enzyme inhibition aspect, it is possible that the monoclonal VAP-1 antibody discussed above may well be closer to the clinic. The task of designing small molecules to interfere with cell-cell recognition is certainly feasible, but this will not be a trivial effort, more akin to the search for selective selectin antagonists which has proven to be very challenging. As confidence grows in the pharmaceutical industry that SSAO/VAP-1 is a validated target, it is inevitable that considerable resources will be directed to all avenues to block the functional action of this protein.

Treatment of A1 AD lung disease is a much simpler task, involving simple sustained augmentation of serum levels. In moving toward that goal, pre-clinical work supports the concept of intramuscular administration of rAAV2-AAT. Reports from mouse, rabbit, and baboon models all indicate safety and bioactivity of this vector (Song et al., 1998, 2001, 2002 Poirier et al., 2004). Based upon that data, a phase I clinical trial in AAT deficient adults was recently initiated (Flotte et al., 2004). 

Removing residual transition metals after reaction completion is a major task for many chemists in the pharmaceutical industry. A second step that can be taken is to actually have the physician or a representative from the clinic come to the school for a tour. This will provide an opportunity to clearly explain the physical requirements for each primary job, as well as to review the degree to which transitional return-to-work tasks have been identified that can be used to immediately return an employee to work, regardless of an employee s primary job. Developing this level of relationship with treating physicians will allow them to make more informed decisions on how to communicate the status of an employee following the treatment of an injury. For example, an uninformed physician may immediately place an employee off of work for three days to heal from an injury. However, if that same physician was aware that a mature transitional return-to-work program existed, the employee might be returned to a role that accommodates work restrictions that have been placed on the employee.

A host of carriers, with a wide variety of ion selectivities, have been proposed for this task. Most of them have been used for the recognition of alkali and alkaline metal cations (e.g., clinically relevant electrolytes). A classical example is the cyclic depsipeptide valinomycin (Fig. 5.13), used as the basis for the widely used ISE for potassium ion (38). This doughnut-shaped molecule has an electron-rich pocket in the center into which potassium ions are selectively extracted. For example, the electrode exhibits a selectivity for K+ over Na+ of approximately 30,000. The basis for the selectivity seems to be the fit between the size of the potassium ion (radius 1.33 A) and the volume of the internal cavity of the macrocyclic molecule.
