Most anticancer drugs damage hair follicles and produce partial or complete alopecia. Patients should be warned of this reaction, especially if paclitaxel, cyclophosphamide, doxorubicin, vincristine, methotrexate, or dactinomycin is used. Hair usually regrows normally after completion of chemotherapy. 

Screening of microbial products has led to the discovery of a number of growth inhibiting compounds that have proved to be clinically useful in cancer chemotherapy. Many of these antibiotics bind to DNA through intercalation between specific bases and block the synthesis of RNA, DNA, or both cause DNA strand scission and interfere with cell replication. All of the anticancer antibiotics now being used in clinical practice are products of various strains of the soil microbe Streptomyces. These include the anthracyclines, dactinomycin, bleomycin, and mitomycin.

Dactinomycin [dak ti noe MYE sin], known to biochemists as actino-mycin D, was the first antibiotic to find therapeutic application in tumor chemotherapy.

Items 80-81. A patient with metastatic choriocarcinoma was ti eated first with methotrexate plus dactinomycin and subsequently with a combination of cisplatin and vincristine. In both regimens, drug dosage was maximized to a toxicity limit of a 2-log decrease in blood platelets. The effects of chemotherapy were monitored by urinary chorionic gonadotropin (UCG, U/24 h), as shown in the data below. 

According to SIOP protocols, treatment is based on preoperative chemotherapy to reduce the risk of peroperative tumor rupture and reduce the local stage (Lemerle et al. 1976 Tournade et al. 1993 Godzinski et al. 1999 2001 Tournade et al. 2001). Chemotherapy regimens consist of dactinomycin, vincristine doxorubicin. Conversely, the NWTS approach is based on primary nephrectomy. Total nephro-ureterectomy remains the surgical reference treatment.
