Benign biliary ductular proliferations are seen in a variety of conditions associated with hepatic injury and subsequent metaplastic changes. Bile duct hamartomas (von Meyenburg complexes) are small collections of bile ductules displaying dilated ductules with contour irregularities, single layer of bland, cuboidal epithelium, and intraluminal bile.

Gadolinium chelates some authors observed homogeneous enhancement of these lesions, some others a thin rim enhancement, whereas others did not find any enhancement (Mortel and Ros 2001 Maher et al. 1999 Martinoli et al. 1992 Murphy et al. 1989 Slone et al. 1993 Wohlgemuth et al. 1998). At MR cholangiography, bile duct hamartomas appear as multiple tiny cystic lesions that do not communicate with the biliary tree, helping in the differential diagnosis with Caroli s disease (Fig. 7.6b-d) (Luo et al. 1998).

Bile-duct adenomas are rare. Usually, they are solitary and < 2 cm in size. This type of adenoma is more often detected in men than in women (3 1) and it occurs in patients mainly over the age of 50. It is nearly always localized beneath the liver capsule. This adenoma has the form of a firm, whitish node and consists of bile-duct proliferations. It contains biliary acini and tubules which are lined with a layer of cuboid epithelium within loose fibrous, partially hyaline stroma. There is evidence of mononuclear inflammatory cells and, occasionally, lymph follicles. It is discussed that such biliary adenomas are peribiliary gland hamartomas. (162) There are no clinical or biochemical abnormalities, nor is there any tendency towards malignant degeneration. (161,163)... 

Benign biliary microhamartomas (= Meyenburg s complex) were described by H. von Meyenburg in 1918. They consist of small cysts developing from dilatations of the small (interlobular) bile ducts and are surrounded by fibrous stroma. The cysts are remnants of ductal plate malformations. Mesenchymal hamartomas are considered to be a developmental anomaly more than true neoplasm. A total of 75% of cases are located in the right lobe of the liver. Histologically MHR arise from the mesenchyma of the portal tract and will have connective tissue, blood vessels, lymphatic spaces, bile ducts and normal hepatocytes (Konez et al. 2001 Cetin et al. 2002). 

These lesions include simple (bile duct) cyst, autosomal dominant polycystic liver disease, biliary hamartoma, Caroli s disease, pyogenic and amoebic abscesses, intrahepatic hydatid cyst, biliary cystadenoma and cystadenocarcinoma, cystic subtypes of primary liver neoplasms, cystic metastases, and intrahepatic hematoma and biioma.
