Barbiturate paracetamol

Fetal microcephaly has been attributed to cocaine abuse during pregnancy (323). Urine toxicology confirmed the presence of morphine, benzoylecgonine, barbiturates, paracetamol, and propoxyphene. Analyses of amniotic fluid, placenta, and fetal serum and urine were also positive for these substances. The authors suggested that vascular disruption was the likely major mechanism of anomalies, both behavioral and malformative, due to prolonged exposure to cocaine in utero.

Although investigation should always precede analysis, there is no need to wait for full completion of tile questionnaire because some poisons are so common that they should always be eliminated. These include ethanol, aspirin, paracetamol, barbiturates, and carbon monoxide. Many of tiiese can be checked by rapid analytical probing tests before a full group screen is applied. Completion of the following analytical probing tests will, in many cases, provide clues to the nature of the poison. 

The outcome, however, can depend on the individual patient. The recommended dose of paracetamol is i to 2 tablets (500 mg each) but the size of dose necessary to cause damage depends on the individual. The average individual who is not taking other drugs regularly, may have to take about twenty tablets to suffer liver damage, but with some individuals as few as ten tablets can have the same effect. There is individual variability in the sensitivity to paracetamol due to variation in the metabolism. A major factor is the use of alcohol and certain other drugs such as barbiturates (see case notes). 

Barbiturates and alcohol both increase the activity of (induce) the enzymes that metabolize paracetamol to the toxic product. A chronic alcohohc or someone who has been prescribed barbiturate drugs will be at greater risk when taking more than the recommended dose of paracetamol because a greater proportion of the dose will be converted into the toxic product. It would be equivalent to taking a larger dose. In the case of an alcoholic the liver may already be compromised by repeated and long-term alcohol abuse and so be more vulnerable and less able to detoxify paracetamol it may also have less protective substances like thiols. 

In New 2 ealand, the 1984 Medicines Regulations require that the 6 types of medicines that were most frequently associated with potentially serious poisoning incidents in children be supplied in strip or blister packaging which is reasonably resistant to attempts of young children to gain access to contents. These medications arc aspirin, paracetamol, iron salts, barbiturates, phenothiazines, and antidepressants. Liquid forms of those medicines may be dispensed with a container fitted with a CRC. It should be noted that this list is now somewhat dated, given that barbiturates are not now generally available in New Zealand. 

The UK manufacturers report that in interaction clinical studies there was no evidence of clinically relevant interactions between letrozole and other commonly prescribed drugs, namely benzodiazepines such as diazepam, barbiturates, diclofenac, fiirosemide, ibuprofen, omeprazole, and paracetamol (acetaminophen). ... 

Examples of substances that are prone to hydrolysis are acetylsalicylic acid, ampicillin, barbiturates, chloramphenicol, chlordiazepoxide, cocaine, corticosteroid phosphate or succinate esters, proteins, folinic acid, indomethacin, local anaesthetics, paracetamol (acetaminophen), pilocarpine, tropa alkaloids (atropine, scopolamine), xylomethazoline and the antimicrobial preservatives methyl and propyl parahydroxybenzoate. In the field of oncology, melphalan and bendamustine hydrochloride are highly susceptable to hydrolysis with a shelf life of 1.5 h for melphalan and 3.5 h for bendamustine at room temperature.
