The patient shown in Figure 8.22, a Caucasian with Fitzpatrick skin type III, was treated with a 1-minute Stone II peel followed by waterproof tape occlusion for 48 hours and 3-day blending resorcinol peel to neck. (The face was treated with Aquaphor ointment after tape removal, as was the neck after the 3rd day of five coats of resorcinol - one every 5 minutes for five coats on three consecutive days. The resorcinol is put on like glycolic acid, one even coat every 5 minutes for five coats and repeated 3 days in a row. On the 3rd day after the five coats of resorcinol have dried, either thymol iodide powder is applied to form a crust or Aquaphor ointment is applied throughout healing. Either occlusion method works well. 

Once the desired frost is achieved, the skin can be rinsed off with water, or cooled down with cool wet compresses which are applied to the skin. The wet compresses can provide a welcome relief to the burning induced by the peel. Unlike glycolic peels the water does not neutralize the peel, as the frosting indicates the end-point of the reaction rather, it dilutes any excess TCA. The compresses can be repeated several times until the burning sensation has subsided. Subsequently, a layer of ointment such as plain petrolatum or Aquaphor is applied and post-peel instructions and what to expect are reviewed with the patient prior to discharge to home. 

After these initial measures, patients are instructed to apply any plain ointment like petrolatum or aquaphor plus broad spectrum sunscreens (physical blocks, such as zinc oxide + titanium dioxide). Once healed, topical bleaching agents and broad spectrum sunscreens are re-started. 

At the conclusion of the procedure, Aquaphor or petrolatum ointment is applied to treated areas. Patients are instructed to cleanse these areas with a gentle cleanser and may apply bland moisturizer twice daily to promote healing and hasten exfoliation. Sun avoidance in the immediate postprocedure period and broad spectrum photo protection thereafter are recommended. From the outset, patients are advised that multiple sessions (3-6 in most people) are needed to see substantive improvement.
