More recently, the challenge to formulate with consistency highly potent actives present in diminishingly low concentrations raised the bar for formulations another significant notch. Procedures and devices for safe intravitreal implantation of sustained antiviral medication have grown from the advent of new therapies for a life- and eye-threatening new disease, HIV-AIDS.

It may appear that vaccines are sui generis and that other pharmaceuticals do not exhibit the same mix of public benefits and costs. That is not so. Antibiotics and antiviral medications raise many of the same issues as vaccines. Like vaccines, they can provide protection for individuals other than the patient. For example, numerous studies have shown that the transmission of HIV from mother to fetus can be dramatically reduced if the mother is treated with an antiviral near the time of delivery (American Academy of Pediatrics 2004). Likewise, studies have suggested that the reduction of viral load that can result from effective antiviral treatment for HIV can reduce the infectiousness of an HIV positive individual (Scheer et al. 2001). 

The third patient developed the infection after 1 month the keratitis cleared on withdrawal of latanoprost and antiviral therapy reinstitution of latanoprost with prophylactic antiviral medication (valaciclovir) kept the cornea clear, but as soon as the antiviral drug was discontinued, //. simplex virus keratitis reappeared. 

A 37-year-old HIV-infected woman receiving stavudine, lamivudine, and indinavir developed epigastric pain, anorexia, and vomiting. She had lactic acidosis (serum lactate 4.9 mmol/1), raised liver enzymes, and an increased prothrombin time. She had hepatomegaly and tachypnea and required mechanical ventilation. Her progress was complicated by pancreatitis and acute respiratory distress syndrome. Antiviral medication was stopped and she was treated with co-enzyme Q, carnitine, and vitamin C. The serum lactic acid and transaminases returned to normal over 4 weeks and she was weaned off the ventilator after 4 months.

CMV retinitis is the most common opportunistic eye infection in patients with AIDS and immunocompromised transplant patients. Antiviral medications used in the treatment of CMV are generally administered in two stages induction therapy, to achieve disease regression, followed by maintenance therapy. The incidence of CMV retinitis has decreased significantly with the advent of HAART for AIDS, and antiviral therapy for CMV may often be discontinued in patients who respond favorably to HAART and achieve an elevation in CD4 cell levels above lOO/pl. Noninfectious indolent epithelial ulcers also can occur in HSK.These ulcers, formerly referred to as metaherpetic lesions, tend to be ovoid, 2 to 8 mm in size, with smooth rolled edges. They may be caused by damage to the epithelial basement membrane due to inflammation, tear film abnormalities, neurotropic cornea, or toxicity from antiviral medications. These ulcers may be recalcitrant, resulting in neovascularization and scarring. 

Antivirals are toxic to the corneal epithelium and may delay healing, so their frequency of use should be decreased after the first week to five times a day.Therapy should be continued for several days after healing to allow time for the dormant virus to be shed. Some authors suggest tapering antiviral medications.

Nervous system Neurotoxicity in adult dialysis patients secondary to valaciclovir continues to be reported in case reports [35, 37, 50 ] and observational studies [51 j. These include a case where a patient was felt to have status epilepticus due to aciclovir and valaciclovir neurotoxicity [37 ]. A Cochrane review of antiviral medications for preventing CMV disease in SOT recipients foxmd neurological dysfxmction to be more common with GCV and valaciclovir compared with placebo or no treatment [52 ].

Warfarin was also implicated in several interactions with antiviral medications. A recent case series suggests an interaction between warfarin and oseltamivir (Tamiflu). Among 15 patients on anticoagulation tiierapy who took oseltamivir, 7 had an increase in INR levels and the remaining 8 maintained stable INR levels. Bleeding events such as blood-tinged sputum and bloody ascites occurred in three patients in the increased INR group [13 ].
