Androgens are used medically as replacement therapy in male hypogonadal disorders (i.e. impaired functioning of the testes). They have a low incidence of adverse reactions and the reactions that occur are generally mild. Rapid intravenous infusion of H2 antagonists may cause bradycardia. Cimetidine is more inclined to cross the blood-brain barrier and CNS effects such as somnolence and confusion have been described, especially in the elderly and in patients with impaired renal function. Cimetidine in high doses, as the only one of its class, has antiandrogenic effects which could be explained by an increase of prolactin secretion, binding to androgen receptors and inhibition the cytochrome P450 mediated hydroxylation of estradiol.

A 43-year-old man developed impaired vision in both eyes over 3 months. Anterior subcapsular opacities were found in both eyes, necessitating cataract extraction. He had been taking finasteride 1 mg/day for 3 years to treat the early stage of androgenic alopecia. It was suspected that the drug was responsible and the treatment was therefore withdrawn. 

Replacement Therapy. Testosterone and other androgens are administered as replacement therapy when the endogenous production of testosterone is impaired. By contrast, deficiency of 17-hydroxylase results in impaired ability of the gonads (as well as the adrenals) to synthesize androgens (males) or estrogen (females) patients subsequently manifest with problems typically associated with primary hypogonadism. Deficiency of 17-hydroxylase is similar to that of 11 (3-hydroxylase in that there is adrenal overproduction of min-eralocorticoids, resulting in sodium retention and high blood pressure. Differential diagnosis is usually facilitated by examination of the complete profile of adrenocortical steroid hormones.
