Ancillary Tests

A medical history, physical examination, PFTs, and HRCT should be performed on all patients with suspected DPLD. In many cases, this will provide sufficient data for a diagnosis. In others, the diHerential diagnosis will require additional ancillary testing. The most common of these procedures are bronchoscopy, serologies, and surgical lung biopsy.

An ancillary test involved examining discharge residue particles from the ammunition used in the mercury distribution tests. Approximately 260 particles were examined of which 8 contained mercury (about 3%) at minor or trace level. 

Ancillary testing to determine any deleterious side effects of inhibitor additions on circuit function.

Ancillary Testing of Inhibitor Resin Properties. It was found that inhibitor could be introduced fairly readily in the two part liquid system either by direct dissolution in the resin or hardener or, for some of the less soluble inhibitors, as a fine dispersion in the resin. Introduction in powder transfer moulding resins was achieved by ball milling the inhibitor with the powder. Subsequent analysis of samples of powder showed that the inhibitor had been uniformly dispersed. 

Six experiments with the Cf( 0,4n) Sg reaction were performed with a total number of ions (called beam dose ) between 0.8 x 10 and 2.6 x 10 in each case. In the first two thermochromatographic runs, a quartz wool filter was present in the start zone as in most ancillary tests. No fission tracks were found in the column. In the four following experiments, this filter was omitted. In the fifth experiment, no chlorinating reagents were added and the column was kept at ambient temperature to test the behavior of the nonvolatile actinides. 18 SF events were detected within the first 15 cm of the column. The number of tracks found in runs 3, 4, and 6 are 10, 20, and 11, respectively, and these are mostly distributed along the first 20-120 cm of the column, see Fig. 20.33. 

There are multiple objectives associated with each follow-up evaluation. These may vary from one session to another and will determine the relative complexity of the particular session whether ancillary testing such as an echo-Doppler evaluation or chest x-ray is indicated or if the sole test that is required is a simple ECG rhythm strip. 

On occasion, office-based testing may not be adequate and ancillary tests will be required to complete the evaluation. 

The blood lead level at present remains the single most important test to monitor lead exposure and is the test used in the medical surveillance program under the lead standard to guide employee medical removal. The ZPP has several advantages over the blood lead level. Because of its relatively recent development and the lack of extensive data concerning its interpretation, the ZPP currently remains an ancillary test.

Tables 4 and 5 summarize the general applications of mutagenesis assays. Phase I involves utilization of the assays as primary detection, prioritization, and risk-assessment methods, and Phase II illustrates their application as ancillary tests for quality control and occupational monitoring.
