An analog of the above "reversed" pethidine, alphaprodine (114), has found application as an analgesic in the clinic. 

The ring-contracted analog of alphaprodine is prepared by a variation of the scheme above. Alkylation of 109 with ethyl bromoacetate affords the lower homolog diester (115). Dieckmann cyclization followed by saponification-decarboxylation yields the pyrrolidine (116). Reaction with phenylmagnesium bromide leads to the condensation product (117) acylation with propionic anhydride gives the analgesic agent prolidine (118)...

Extensive molecular dissection of the morphine molecule over the past several decades led to a host of molecules which showed narcotic analgesic activity even though they possessed but faint suggestion of the structural features present in morphine itself. Narcotics are divided into naturally occurring, semisynthetic, and synthetic derivatives. The naturally occurring analgesics consist of morphine and codeine (methylmorphine). The semisynthetic analgesics include hydromorphone (Dilaudid) and hydrocodone (Dicodid). The synthetic analgesics consist of meperidine (Demerol), alphaprodine (Nisentil), methadone (Dolophine), propoxyphene (Darvon), and pentazocine (Talwin). 

Alphaprodine, l,3-dimethyl-4-phenyl-4-piperidyl propionate hydrochloride (Nisentil , Prisil-idene ), resembles meperidine chemically. Its analgesic and depressant actions are less intense, but more prompt and of shorter duration, than those of morphine. It is a potent, short-acting narcotic... 

Alphaprodine hydrochloride is administered by subcutaneous injection in doses of 40 to 60 mg depending upon the weight of the patient. Analgesia occurs within 5 min and lasts about 2 h. Nalorphine is an effective antidote to overdosage.
