Twin pregnancy

Follitropin treatment for infertility has been reviewed in detail (1,2). Multiple pregnancy occurs in 20% of patients, 80% being twin pregnancies. There is no documented increase in congenital abnormalities in children conceived after ovulation induction with follitropin.

Significant neonatal morbidity can occur after transabdominal infusion of methylthioninium chloride to diagnose premature rupture of fetal membranes, to stain the amniotic fluid in twin pregnancies, or after postpartum administration of methylthioninium chloride. Toxic manifestations include hyperbilirubinemia, Heinz body hemolytic anemia, and possible desquamation of the skin. In most cases it appears that toxicity was the result of an overdose of methylthioninium chloride (6-9). 

Multiple ileal occlusions have been reported in babies born to mothers who had twin pregnancies and who had received methylthioninium chloride administered during amniocentesis (10-12). In a number of cases it was found that methylthioninium chloride had been injected into the amniotic sac of the affected twins.

Calculation of Down syndrome risk requires that the distribution of analyte values for all tests in both unaffected and affected pregnancies be known. These distributions are well defined for AFP, uEs, CG, and DIA in singleton pregnancies, and thus reliable risks can be calculated. More than 50% of femoral fractures are simple transverse, closed and non-comminuted (Rockwood 1991). The middle third is the most frequent site of fracture (70%), followed by the proximal third and lastly the distal third. Femoral fractures occurring before the child is able to walk are strongly associated with child abuse (Gross and Stranger 1983). Birth related fractures are exceedingly rare (0.13 per 1000) and usually associated with twin pregnancies, breech presentation or foetal osteoporosis (Morris et al. 2002).
