A small amount of dimethylamine is always present in the crude product. This is removed before collection of the product by operating the column under total reflux for 1-2 hours. Rabbit Eye Irritation. Instillation of the chlorodibenzodioxins into the conjunctival sac caused slight, transient pain and conjunctival inflammation, initially. There is, however, a unique risk in the bipolar form that antidepressant treatment may trigger a switch into mania. This may occur either as the natural outcome of recovery from depression or as a pharmacological effect of the drug. Particular antidepressants (the selective serotonin reuptake inhibitors) seem less liable to induce the switch into mania than other antidepressants or electroconvulsive therapy. Treatment for mania consists initially of antipsychotic medication, for instance the widely used haloperidol, often combined with other less specific sedative medication such as the benzodiazepines (lorazepam intramuscularly or diazepam orally). The manic state will usually begin to subside within hours and this improvement develops further over the next 2 weeks. If the patient remains disturbed with manic symptoms, additional treatment with a mood stabilizer may help. [Pg.71]

In the initial stages of acute pain treatment, analgesics should be given around the clock. As the painful state subsides, as-needed schedules can be used. Around-the-clock administration is also useful for management of chronic pain. [Pg.629]

Nasal symptoms The effectiveness of intranasal cyanocobalamin in patients with nasal congestion, allergic rhinitis, and upper respiratory tract infections has not been determined. Defer treatment until symptoms have subsided. [Pg.72]

Extrapyramidal symptoms Extrapyramidal symptoms, manifested primarily as acute dystonic reactions, occur in approximately 0.2% to 1% of patients treated with the usual adult dosages of 30 to 40 mg/day. These usually are seen during the first 24 to 48 hours of treatment, occur more frequently in children and young adults, and are even more frequent at the higher doses used in prophylaxis of vomiting caused by cancer chemotherapy. If symptoms occur, they usually subside following 50 mg diphenhydramine IM. Benztropine 1 to 2 mg IM may also be used to reverse these reactions. [Pg.1394]
