Tolerance, to drugs

ECT continues to be used at a higher frequency in geriatric versus younger patients. This difference in usage probably reflects the decreased efficacy of medications seen in older patients, a lower tolerance to drug side effects, and the higher incidence of medical disorders in this age group ( 1). 

The election of Jimmy Carter as President in 1976 ensured a continuation of tolerance to drug use, particularly marijuana. In March 1977, the Special Assistant for Health Issues to the President, and high officials from the DEA, the State Department, the National Institute of Drug Abuse (NIDA), NIMH, the Customs Service, and the Justice Department appeared before the House Select Committee on Narcotics Abuse and Control to argue for the decriminalization of marijuana. The President himself repeated a similar theme before Congress later that year.

Tolerance to drug therapy may result in and provide an explanation for suboptimal treatment, e.g. with an antiepilepsy drug. 

It is believed that tolerance to drugs such as morphine (i.e. the necessity to take higher doses to achieve the same result) may be due to some adaptation whereby the receptor/ligand complex is less efficient in activating the Gj protein. As a result, larger levels of drug are necessary. 

Symptoms of dizziness and syncope due to postural hypotension may result from dehydration, blood loss, and myocardial disease. However, the most important cause of chronic recurrent symptomatic postural hypotension is failure of the autonomic nervous system. The drug treatment for postural hypotension is difficult because the response varies and tolerance to drugs develops. Patients with autonomic abnormalities may suffer symptoms after eating, and this further hampers treatment. 

Patients may develop a tolerance to drug effects with chronic use, and a rebound anxiety is also seen in waking hours. Impaired cognitive and motor functions also may occur, particularly in the elderly.

A tolerance to alcohol is developed—Greater amounts must be consumed to obtain the desired effects. Often, the increased tolerance to alcohol is accompanied by increased tolerances to drugs such eis barbiturates.
