Glucose infused into the renal artery spills into urine when the renal threshold is exceeded in the same anomeric form as that administered, whereas reabsorbed glucose in the renal vein is mutarotated. Coronary artery disease remains the main killer. The mortality from coronary artery disease has decreased by 28% in the last 20 years. A first acute myocardial infarction had mortality of 28% in 1963 and today it is about 7%. The price of life has become more and more valuable. We are only on the threshold of understanding the pathogenic and biological principles of the generation of atheroma, but once established, obstructive lesions need coronary artery dilatation, coronary artery bypass or arterial replacement. The saphenous vein is an excellent bypass conduit but its life expectancy is 3 to 10 years only until it thromboses, or becomes atheromatous and is blocked or obliterated by cholesterol filled intima or clot. The internal mammary artery is ideal but flow is too small to sustain adequate flow to the entire heart. Here too, there is the need for new non-thrombogenic materials to reconstruct artificial arteries, substances which are durable, non-thrombotic and do not promote atherogenesis. There is also the need for simple, non-invasive techniques to measure coronary blood flow and the anatomy of the coronary arteries to plan subsequent surgical procedures. [Pg.413]

Looking at safety of these concentrates, analysis of 3 studies found adverse events in 3 of 26 patients receiving fibrinogen concentrate, whereas 12 of the 27 control patients experienced an adverse event [120 ]. The three reported adverse events were subclinical PE, vein graft occlusion and postoperative atrial fibrillation. One of the studies included in this analysis did not report safety data. Going forward, it will be important for studies to consider issues of threshold, clinical situation, role of testing and documentation of adverse events [121 ]. [Pg.493]

For the preoperative evaluation of potential liver transplant recipients in our institution we perform a multiphasic multidetector CT study, which enables the evaluation of parenchyma and accurate assessment of the arterial, portal venous and hepatic venous anatomy. Oral milk or water (500 ml) is used to opacify the proximal bowel. After a non-intrave-nously enhanced scan is performed, 200 ml of nonionic 350 mg I/ml contrast material is administered intravenously at a 5 ml/s rate. The high iodine concentration allows better visualization of the veins. Acquisition timing is guided by bolus tracking in the aorta (enhancement threshold 200 HU) a late arterial phase scan is initiated 10 s after the threshold is reached, then a venous phase Gate portal-hepatic venous) scan is initiated with a 45-s delay from threshold a delayed acquisition is performed after 3 min. [Pg.115]
