Temporomandibular joint

Patients with IBS may experience comorbidities outside the gastrointestinal tract such as fibromyalgia, sleep disturbances, headaches, dyspareunia, and temporomandibular joint syndrome.

A 44-year-old man took amfebutamone 300 mg/day and buspirone 45 mg/day for depression (9). Over 2-3 weeks he developed increasing neck stiffness, with tightening and spasm of the jaw muscles and pain in the left temporomandibular joint. He stopped taking his medications, and all his symptoms resolved over the next 3 weeks. Rechallenge with buspirone (45 mg/day) failed to reproduce the dystonic symptoms. The buspirone was withdrawn and amfebutamone 150 mg/day was started the dystonic symptoms did not recur. However, 24 hours after the dose of amfebutamone was increased to 300 mg/day there was a return of neck stiffness and jaw spasm. 

Temporomandibular joint dislocation has also been reported (203).

Interposition of Teflon-Proplast implants for internal derangement of the temporomandibular joint has been used, although cases of osseous destructive changes have been reported (16).

Other upper 2nd and 3rd molars (fig. 3) show similar values of //and E at the enamel surface to those of the upper 2nd molar shown in figure 1. In all cases the EDJ is noticeably weaker. The temporomandibular joint (jaw) can be affected, resulting in malocclusion and difficulty in chewing food. Inflammation of cartilage in the chest can lead to chest wall pain. Hip pain may occur as a result of destructive changes in the hip joint, soft-tissue inflammation (e.g., bursitis), or referred pain from nerve entrapment at the lumbar vertebrae.

A (Botox , Dysport ) has an Mr of ca. 90(XX) and is used in extremely small doses in ophthalmology to treat idiopathic blepharospasm (squinting) and other spasms such as hemifacial spasm, facial paralysis, TMJ (temporomandibular joint disorder), writer s cramp, extreme sweating, etc.. Recent clinical data show, that B. is effective in the treatment of essential hand tremor and upper limb spasticity after a stroke, as well as in certain cases of headache/migraine. lit J. Temporomandibular joint dislocation due to dystonia after a single dose of aripiprazole has been reported [60" ]. 

An advanced approach to this use of 3D information is the construction from the patient s data of a customised prosthesis (Ray et al. 1993). This allows a replacement prosthesis to be adapted perfectly to the patient s own anatomy and has been developed for replacement of the temporomandibular joint in particular (Worrall and Christensen 2006). 

Internal derangement of the temporomandibular joint is conventionally investigated by MRl, with the objective of demonstrating abnormalities of the meniscus and its relationship to the joint surfaces. 3D imaging has a contribution to make in more severe stages of joint disorganisation and degeneration, and in trauma.
