A shaft rotating at 4000-6000 rev/min carries a primary air fan and an atomizing cup. The cup, typically of about 70-120 mm diameter, is tapered by a few degrees to increase in diameter at the exit. A stationary distributor which projects oil onto the smaller-diameter end of the cup feeds oil to the inner surface. The oil, influenced by centrifugal force, forms a thin film, which passes towards the cup lip. Atomization occurs as the oil leaves this lip. In addition, a primary air supply, normally in the range of 5-12 per cent of stoichiometric (chemically correct) air. 

Capsugel has also developed the Coni-Snap SuproR capsule (Fig. 6). Similar to Coni-Snap in regard to locking mechanism and tapered body edge, this capsule differs in that it is short and squat and the cap overlaps the body to a greater degree [21].

In an attempt to switch the patient to the less expensive L-type CCB verapamil for discharge, she was introduced slowly to the transition on a blind basis and was also unable to maintain her previous degree of clinical improvement with carbamazepine and nimodipine (Figure 6-5C). Maximally tolerated daily doses were 320 mg of verapamil and 600 mg of carbamazepine. In light of this recrudescence of clinical symptomatology, the patient was introduced to the transition on a blind basis back to nimodipine, and substantial recapturing of her clinical responsivity was again observed. Adverse effects. Side effects that occurred with nefazodone to a greater degree than with placebo and that were dose related included somnolence, dry mouth, nausea, and dizziness. Furthermore, there is no need to taper the dose if discontinuation is being considered (L. Friedman et al. 1992 L. H. Gold and Balster 1991]. More than 2,256 patients have been studied in clinical trials, and, to date, only two cases of overdose (>3,600 mg ingested] have been reported both patients recovered with no sequelae. 

Corticosteroids are sometimes used in the treatment of severe symptomatic gout, by intra-articular, systemic, or subcutaneous routes, depending on the degree of pain and inflammation. The most commonly used oral corticosteroid is prednisone. The recommended dose is 30-50 mg/d for 1-2 days, tapered over 7-10 days. Intra-articular injection of 10 mg (small joints), 30 mg (wrist, ankle, elbow), and 40 mg (knee) of triamcinolone acetonide can be given if the patient is unable to take oral medications.
